筋膜松解术联合关节松动术治疗肩周炎的临床研究

Clinical study on the treatment of scapulohumeral periarthritis by fasciolysis combined with joint mobilization

目的研究筋膜松解术联合关节松动术治疗肩周炎的临床效果。方法70例肩周炎患者为研究对象,采用随机数字表法分为试验组和对照组,各35例。试验组采用筋膜松解联合关节松动术治疗,对照组采用蜡疗联合关节松动术治疗。对比两组日常生活活动能力评分、视觉模拟评分法(VAS)评分及肩关节活动功能改善效果。结果试验组进食、洗澡、个人卫生、穿衣评分分别为(8.34±1.23)、(7.42±1.35)、(8.92±0.02)、(8.34±0.32)分,均高于对照组的(6.23±1.33)、(4.23±1.45)、(6.62±0.04)、(6.72±0.02)分,差异有统计学意义(P<0.05)。试验组VAS评分(2.04±0.16)分低于对照组的(4.52±0.34)分,差异有统计学意义(P<0.05)。试验组的肩关节活动功能改善总有效率97.14%高于对照组的71.43%,差异有统计学意义(P<0.05)。结论筋膜松解术联合关节松动术治疗肩周炎患者效果显著,能使患者的日常生活活动能力、肩关节活动功能逐渐增强,疼痛感得到缓解。

Objective To study the clinical effect of fascia lysis combined with joint mobilization in the treatment of scapulohumeral periarthritis.Methods A total of 70 patients with scapulohumeral periarthritis were divided into experimental group and control group according to the random numerical table,with 35 cases in each group.The experimental group was treated with fascia lysis and joint mobilization,and the control group was treated with wax therapy and joint mobilization.The activities of daily living score,visual analogue scale(VAS)score and shoulder joint function were compared between the two groups.Results The scores of eating,bathing,personal hygiene and dressing of the experimental groups were(8.34±1.23),(7.42±1.35),(8.92±0.02)and(8.34±0.32)points,which were higher than those of(6.23±1.33),(4.23±1.45),(6.62±0.04)and(6.72±0.02)points of the control group,and the differences were statistically significant(P<0.05).VAS score of(2.04±0.16)points in the experimental group was lower than that of(4.52±0.34)points in the control group,and the difference was statistically significant(P<0.05).The total effective rate of shoulder joint function improvement in the experimental group was 97.14%,which was higher than that of 71.43%in the control group,and the difference was statistically significant(P<0.05).Conclusion Fasciolysis combined with joint mobilization is effective in the treatment of patients with scapulohumeral periarthritis.It can gradually enhance the activities of daily living and shoulder joint activities of patients,and relieve the pain of patients.
