摘要
目的研究扩散加权成像(DWI)联合容积内插屏气检查(VIBE)动态增强技术对前列腺癌(PCA)的诊断价值。方法选取83例PCA患者作为PCA组,另选取50例男性健康体检者作为对照组。两组均行DWI联合VIBE动态增强技术检查。以病理结果为金标准,分析DWI联合VIBE动态增强技术检查的结果;对比两组表观弥散系数(ADC)值及动态增强磁共振成像(DCE-MRI)参数[感兴趣区转运常数(K^(trans))、血管外细胞外间隙体积百分数(V_(e))及速率常数(K_(ep))]。结果PCA组83例患者经DWI联合VIBE动态增强技术检查检出PCA 82例,检出率为98.80%,误诊1例(1.20%)。PCA组ADC值(1.036±0.159)×10^(-3)mm^(2)/s低于对照组的(1.712±0.154)×10^(-3)mm^(2)/s,V_(e)(0.26±0.04)、K^(trans)(0.36±0.04)/min及K_(ep)(0.73±0.08)/min均高于对照组的(0.17±0.05)、(0.21±0.02)/min、(0.61±0.05)/min,差异具有统计学意义(P<0.05)。结论DWI联合VIBE动态增强技术在PCA中的诊断价值较高,可为临床诊治提供可靠依据。
Objective To study the value of diffusion weighted imaging(DWI)combined with volumetric interpo-lated breath-hold examination(VIBE)dynamic contrast-enhancement technology in the diagnosis of prostate cancer(PCA).Methods A total of 83 PCA patients were selected as PCA group,and 50 healthy male subjects were selected as control group.DWI and VIBE dynamic contrast-enhanced examination were performed in both groups.Pathological findings were used as the gold standard to analyze the results of DWI combined with VIBE dynamic contrast-enhanced examination.Both groups were compared in terms of apparent diffusion coefficient(ADC)values and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)parameters[volume transfer constant(K^(trans)),extracellular-extravascular volume fraction(V_(e))and efflux rate constant(K_(ep))].Results In the PCA group,82 cases of PCA were detected by DWI combined with VIBE dynamic contrast-enhancement examination,the detection rate was 98.80%,and 1 case(1.20%)was misdiagnosed.ADC value of (1.036±0.159)×10^(-3) mm^(2)/sin PCA group was lower than that of(1.712±0.154)×10^(-3) mm^(2)/s in the control group;trans PCA group had V_(e) of(0.26±0.04),K^(trans) of(0.36±0.04)/min and K_(ep) of(0.73±0.08)/min,which were higher than those of(0.17±0.05),(0.21±0.02)/min,and(0.61±0.05)/min in the control group;the differences were statistically significant(P<0.05).Conclusion DWI combined with VIBE dynamic contrast-enhancement technique is of high value in the diagnosis of PCA and can provide a reliable basis for clinical diagnosis and treatment.
