雷替曲塞联合奈达铂治疗复发性子宫颈癌的效果及不良反应分析

Analysis of the effects and adverse reactions of raltitrexed combined with nedaplatin in the treatment of recurrent cervical cancer

目的分析子宫颈癌疾病特点,评价雷替曲塞+奈达铂治疗复发性子宫颈癌的效果、安全性。方法60例复发性子宫颈癌患者,按照化疗方案的不同分为对照组与观察组,每组30例。对照组患者采取紫杉醇+顺铂化疗,观察组患者采取雷替曲塞+奈达铂化疗。比较两组患者临床疗效、不良反应发生情况以及治疗前后生活质量评分。结果观察组客观有效率及疾病控制率分别为76.67%、90.00%,对照组分别为76.67%、86.67%。两组客观有效率及疾病控制率比较,差异无统计学意义(P>0.05)。观察组白细胞下降发生率100.00%高于对照组的50.00%,差异具有统计学意义(P<0.05);两组粒细胞下降、血红蛋白下降、血小板下降、发热、恶心呕吐、腹泻/便秘、转氨酶升高、乏力、肾功能异常、外周神经炎发生率比较,差异无统计学意义(P>0.05)。治疗前后,两组生活质量评分组间比较,差异无统计学意义(P>0.05)。治疗后,两组生活质量评分均高于本组治疗前,差异具有统计学意义(P<0.05)。结论雷替曲塞联合奈达铂治疗复发性子宫颈癌效果显著、患者生活质量高,但是白细胞下降不良反应明显。

Objective To analyze the characteristics of cervical cancer and evaluate the effect and safety of raltitrexed combined with nedaplatin in the treatment of recurrent cervical cancer.Methods A total of 60 patients with recurrent cervical cancer were divided into control group and observation group according to different chemotherapy regimen,with 30 cases in each group.The control group received combined paclitaxel and cisplatin chemotherapy,and the observation group received combined rhatitrexed and nedaplatin chemotherapy.The clinical efficacy,occurrence of adverse reactions and quality of life score before and after treatment were compared between the two groups.Results The objective effective rate and disease control rate were 76.67%and 90.00%in the observation group,and 76.67%and 86.67%in the control group,respectively.There was no statistically significant difference in objective effective rate and disease control rate between the two groups(P>0.05).The incidence of leukopenia in the observation group was 100.00%,which was higher than that of 50.00%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of granulocytopenia,decreased hemoglobin,decreased platelet,fever,nausea and vomiting,diarrhea/constipation,elevated aminotransferase,fatigue,abnormal renal function and peripheral neuritis between the two groups(P>0.05).Before and after treatment,there was no statistically significant difference in terms of quality of life scores between the two groups(P>0.05).After treatment,the quality of life scores in both groups were higher than those before treatment in this group,and the differences were statistically significant(P<0.05).Conclusion The combination of raltitrexed and nedaplatin is effective in the treatment of recurrent cervical cancer,and the patient's quality of life is high,but the adverse reaction of leukopenia is obvious.
