CO_(2)点阵激光与黄金微针治疗面部痤疮萎缩性瘢痕的效果对比

Comparison of effects of carbon dioxide fractional laser and gold microneedle in the treatment of facial acne atrophic scars

目的对比黄金微针与CO_(2)点阵激光在面部痤疮萎缩性瘢痕中的应用效果。方法选取2021年4月至2022年6月,泰康同济(武汉)医院皮肤科64例面部痤疮萎缩性瘢痕患者,采用随机信封法分为观察组、对照组(各32例),对照组采用CO_(2)点阵激光治疗,观察组采用黄金微针射频治疗,两组患者均接受3次治疗。记录患者首次治疗后相关时间指标,比较两组治疗前后瘢痕改善情况[临床痤疮瘢痕评估量表(echelle d’evaluation clinique des cicatrices d’acne,ECCA)加权评分]、疗效及不良反应。结果观察组炎性渗出时间、愈合时间、结痂脱落时间、红斑持续时间均较对照组缩短(t=5.154、4.969、6.696、3.868,P<0.05)。治疗后,观察组、对照组患者ECCA权重评分均较治疗前下降(t=32.158、18.420,P均<0.05),且观察组ECCA权重评分显著低于对照组(t=7.436,P<0.05);观察组总显效率93.75%,显著高于对照组的71.88%(P<0.05)。观察组不良反应总发生率9.38%,低于对照组43.75%,差异有统计学意义(P<0.05)。结论黄金微针与CO_(2)点阵激光均是面部痤疮萎缩性瘢痕治疗的有效方式,其中黄金微针疗效更佳,且术后恢复快、不良反应较少,具有更高的临床应用价值。

Objective To compare the application effects of gold microneedle and carbon dioxide fractional laser in the treatment of facial acne atrophic scars.Methods Sixty-four patients with facial acne atrophic scars were selected between April 2021 and June2022,and were divided into observation group and control group by random envelope method,with 32 cases in each group.The control group was treated with carbon dioxide fractional laser,and the observation group was given gold microneedle radiofrequency treatment,and both groups of patients received 3 times of treatment.The relevant time indicators after the first treatment were recorded,and the scar improvement[echelle d'evaluation clinique des cicatrices d'acne(ECCA)weighted score],efficacy and adverse reactions were compared between the two groups before and after treatment.Results The inflammatory exudation time,healing time,scab shedding time and erythema duration in observation group were shorter than those in control group(t=5.154,4.969,6.696,3.868,P0.05).After treatment,the ECCA weighted scores of patients in observation group and control group were reduced compared with those before treatment(t=32.15818.420,P0.05),and the ECCA weighted score was significantly lower in observation group than that in control group(t=7.436,P0.05)The total markedly effective rate of 93.75%in observation group was significantly higher than 71.88%in control group(P0.05).The total incidence rate of adverse reactions of 9.38%in observation group was significantly lower than 43.75%in control group(P0.05)Conclusion Both gold microneedle and carbon dioxide fractional laser are effective methods for the treatment of facial acne atrophic scars.Gold microneedle has better efficacy,faster postoperative recovery,fewer adverse reactions and higher clinical application value.
