壳多糖酶3样蛋白1在消化性溃疡Hp感染中的表达及其诊断价值

Expression of chitosacc haridase 3-like protein 1 peptic ulcer Hp infection and its diagnostic value

目的探讨壳多糖酶3样蛋白1(CHI3L1)在消化性溃疡幽门螺杆菌(Hp)感染中的表达及其诊断价值。方法选取2020年5月-2021年5月杭州市西溪医院收治的80例消化性溃疡患者作为研究组,并依照患者是否合并Hp感染分为未感染组(39例)、Hp感染组(41例)两个亚组。另外同期选取于医院行胃镜检查未发生消化性溃疡的健康志愿者40名作为对照组;使用酶联免疫吸附试验法测定血清CHI3L1水平,分析CHI3L1在各组的水平变化;分析CHI3L1与Hp感染组患者不同临床特征的关系,使用受试者工作特征(ROC)曲线分析CHI3L1对消化性溃疡Hp感染的诊断价值。结果Hp感染组CHI3L1高于未感染组和对照组(P<0.05);Hp感染Ⅱ型、复合性溃疡、活动期患者CHI3L1高于Hp感染Ⅰ型、胃溃疡、十二指肠溃疡、愈合期、疤痕期患者(P<0.05);胃溃疡、十二指肠溃疡患者CHI3L1差异无统计学意义;愈合期患者CHI3L1高于疤痕期患者(P<0.05);CHI3L1对消化性溃疡Hp感染发生的曲线下面积为0.954。结论CHI3L1在消化性溃疡Hp感染中表达升高,且与患者感染类型、溃疡部位、疾病严重程度相关,可用于临床疾病诊断。

OBJECTIVE To investigate the expression of chitosacc haridase 3-like protein 1(CHI3L1)in H.pylori(Hp)infection in peptic ulcer and its diagnostic value.METHODS Totally 80 patients with peptic ulcer admitted to Hangzhou Xixi Hospital from May 2020 to May 2021 were selected as the study group,and were divided into two subgroups,the uninfected group(39 cases)and the Hp infected group(41 cases)according to whether the patients were complicated with Hp infection.In addition,40 healthy volunteers who underwent gastroscopy without peptic ulcer were selected as the control group.The serum CHI3L1 levels were measured by enzyme-linked immunosorbent assay,and changes of CHI3L1 were analyzed in each group;the relationship between CHI3L1 and different clinical characteristics of patients in Hp-infected group was analyzed,and the diagnostic value of CHI3L1 for peptic ulcer Hp infection was analyzed using receiver operating characteristic(ROC)curves.RESULTS The CHI3L1 levels was higher in the Hp infected group than in the uninfected and control group(P<0.05).The CHI3L1 levels higher in patients with Hp infection type II,complex ulcer,active stage than in patients with Hp infection type I,gastric ulcer,duodenal ulcer,healing stage and scar patients(P<0.05).The difference in CHI3L1 between patients with gastric ulcer and duodenal ulcer was not statistically significant.The CHI3L1 levels was higher in patients with healing stage than in patients with scarring(P<0.05).The area under the curve of CHI3L1 for the occurrence of Hp infection in peptic ulcer was 0.954.CONCLUSION The expression of CHI3L1 was elevated in Hp infection in peptic ulcer and correlated with the types of infection,ulcer sites,and severity of diseases,which could be used for clinical disease diagnosis.
