血清相关因子水平变化与寻常型银屑病患者PASI评分关系及临床意义

The relationship between the level of serum related factors and PASI score in patients with psoriasis vulgaris and its clinical signifificance

目的探讨血清血管生成素样蛋白2(ANGPT2)、人软骨糖蛋白39(YKL-40)、血管内皮细胞钙黏蛋白(VE-cadherin)水平变化与寻常型银屑病患者银屑病皮损面积及严重程度指数(PASI)评分关系及临床意义。方法选取2021年2月-2023年2月我院收治的85例(发作期)寻常型银屑病患者为病例组,同期85例健康体检者为对照组。比较2组入院时血清ANGPT2、YKL-40、VE-cadherin水平,分析其与PASI评分、疾病分期的相关性;比较不同病情程度及不同分期患者入院时血清各指标水平,ROC进行效能分析。结果入院时病例组血清ANGPT2、YKL-40、VE-cadherin水平高于对照组,血清ANGPT2、YKL-40、VE-cadherin与PASI评分、疾病分期呈正相关(P<0.05);入院时不同病情程度、不同分期患者血清ANGPT2、YKL-40、VE-cadherin水平比较,重度>中度>轻度,活动期患者>稳定期患者(P<0.05);血清ANGPT2、YKL-40、VE-cadherin联合诊断活动期寻常型银屑病的AUC大于各指标单独诊断。结论血清ANGPT2、YKL-40、VE-cadherin在寻常型银屑病发病中起重要作用且与PASI评分呈正相关,对银屑病的诊断及病情判定具重要指导意义。

Objective To investigate the relationship between serum levels of ANGPT2,human chondroglycoprotein 39(YKL-40),vascular endothelial cell cadherin(VE-cadherin)and psoriasis area and severity index(PASI)scores in patients with psoriasis vulgaris and its clinical signifificance.Methods A total of 85 patients with psoriasis vulgaris admitted to our hospital from February 2021 to February 2023 were selected as case group,and 85 healthy subjects were selected as control group during the same period.The serum levels of ANGPT2,YKL-40 and VE-cadherin were compared between the two groups at admission,and the correlation with PASI score and disease stage was analyzed.The levels of serum indexes of patients with difffferent severity of disease and difffferent stages at admission were compared,and the effffectiveness of ROC was analyzed.Results The serum levels of ANGPT2,YKL-40 and VEcadherin in the case group were higher than those in the control group at admission,and the serum ANGPT2,YKL-40 and VE-cadherin were positively correlated with PASI score and disease stage(P<0.05).The levels of serum ANGPT2,YKL-40 and VE-cadherin in patients with difffferent disease degrees and stages were compared at admission,severe>moderate>mild,stable>active(P<0.05);The AUC of serum ANGPT2,YKL-40 and VE-cadherin in the diagnosis of active psoriasis vulgaris was higher than that of each indicator alone.Conclusion Serum ANGPT2,YKL-40,and VE-cadherin play an important role in the pathogenesis of psoriasis vulgaris and positively correlate with the PASI score,which is an important guideline for the diagnosis of psoriasis and the determination of the disease.
