慢性便秘患者结肠镜检查前肠道准备质量的影响因素及护理对策

目的:探讨慢性便秘患者结肠镜检查前肠道准备质量的影响因素,提出护理对策。方法:选取2018年5月至2021年5月于我院接受结肠镜检查的980例慢性便秘患者进行调查,记录肠道准备情况,据此将患者分为肠道准备合格组和不合格组,应用单因素分析法和多因素Logistic回归分析法分析影响患者肠道准备质量的因素,并进一步根据分析结果提出护理对策。结果:980例患者中105例肠道准备不合格(10.71%)。单因素分析显示,性别、文化程度、饮酒、吸烟、高血压病史、结肠镜检查史方面不同分组患者肠道准备情况无明显差异,P>0.05;年龄、便秘病程、糖尿病史、腹部手术史、服药期间活动情况方面不同分组患者肠道准备情况差异具有统计学意义,P<0.05。多因素Logistic回归分析显示,高龄(年龄>60岁)、糖尿病史、服药期间活动不足是慢性便秘患者肠道准备质量的独立危险因素,P<0.05。结论:慢性便秘患者结肠镜检查前肠道准备质量的危险因素有高龄、糖尿病史、服药期间活动不足等,临床工作中应对具有上述危险因素的患者加强肠道管理,保证肠道准备合格。

Objective To search the factors influencing bowel preparation quality(BPQ)before colonoscopy for chronic constipation(CC)patients,and put forward nursing copy.Methods To investigate 980 CC patients inspected in author's hospital(2018-05-2021-05),record their intestitnal preparation status according to which divide them into qualified group and no-qualified group,then,take mono-and multi-variate Logistic regression methods analyse the factors influencing intestinal preparation quality,further put forward nursing copy.Results In the 980 cases.105 were of qualified group(10.71%).Mono-variate analysis showed between patients'gender,educated levels,drinking,smoking,hypertension history,and colonoscopy history there were no significant difference(P>0.05);between patients'age,constipation history,abdomen surgery history,and activity status during medication oral,there were significant differences(P<0.05).Multi-variate Logistic regression analysis showed advanced age(>60 yr.),DM history,and less(not enough)activity when oral medication were of independent risk factors influencing intestinal preparation quality(P<0.05).Conclusion Advanced age,DM history,and less activity when oral medication are the risk factors influencing pre-colonoscopy intestinal preparation quality,thus it should be to strength,clinically,intestinal management to the patients having above factors,so that to ensure the preparating being qualified.
