关节镜下双滑轮结合缝线桥技术在肩袖损伤治疗中的应用效果 被引量:3

Application of double-pulley combined with suture bridge technique under arthroscope in the treatment of rotator cuff injury

摘要 目的探讨关节镜下双滑轮结合缝线桥技术在肩袖损伤治疗中的应用效果。方法抽取2020年2月至2022年2月临沂市中医医院收治的肩袖损伤患者82例,按照随机数字表法分为观察组和对照组,每组41例。两组均行关节镜手术治疗,观察组予以双滑轮结合缝线桥技术治疗,对照组予以传统缝线桥技术治疗。随访6个月,比较两组术后疼痛评分、肩关节功能、肩关节活动度、日常生活能力及肩袖再撕裂发生情况。结果观察组术后视觉模拟评分法评分[(1.25±0.27)分]低于对照组[(1.78±0.29)分],美国肩与肘协会评分系统评分[(80.63±6.28)分]、Constant肩关节评分[(83.41±6.42)分]、美国加州大学肩关节评分系统评分[(30.41±2.05)分]高于对照组[(72.34±6.15)、(75.12±6.37)、(26.41±2.87)分],P<0.05。观察组术后肩关节前屈度[(145.21±10.32)°]、外旋度[(47.42±4.16)°]高于对照组[(132.47±10.15)°、(40.25±4.04)°],P<0.05。观察组术后Barthel指数量表评分[(90.35±3.58)分]高于对照组[(82.34±6.42)分],P<0.05。观察组肩袖再撕裂发生率(0.00%,0/41)低于对照组(14.63%,6/41),P<0.05。结论关节镜下双滑轮结合缝线桥技术治疗肩袖损伤效果更佳,可减轻患者肩关节疼痛,加快肩关节功能恢复,提高日常生活能力评分,降低肩袖再撕裂风险。 Objective To investigate the application of double-pulley combined with suture bridge technique under arthroscope in the treatment of rotator cuff injury.Methods A total of 82 patients with rotator cuff injuries admitted to Linyi Traditional Chinese Medicine Hospital from February 2020 to February 2022 were selected,and the patients were divided into observation group and control group according to the random number table method,with 41 cases in each group.Both groups underwent arthroscopic surgery,while the observation group was treated by double-pulley combined with suture bridge technique,and the control group was treated by traditional suture bridge technique.Patients were followed up for;the postoperative pain score,shoulder joint function,shoulder joint range of motion,activities of daily living and incidence of rotator cuff re-tear were compared between the two groups.Results The postoperative visual analogue scale score of the observation group(1.25±0.27)was lower than that of the control group(1.78±0.29),P<0.05;the American Shoulder and Elbow Association scoring system score(80.63±6.28),the Constant shoulder joint score(83.41±6.42),and the University of California shoulder joint scoring system score(30.41±2.05)of the observation group were higher than those of the control group(72.34±6.15,75.12±6.37,26.41±2.87)after operation(P<0.05).The postoperative shoulder forward flexion(145.21°±10.32°)and external rotation(47.42°±4.16°)of the observation group were higher than those of the control group(132.47°±10.15°,40.25°±4.04°),P<0.05.The postoperative Barthel index scale score(90.35±3.58)in the observation group was higher than that in the control group(82.34±6.42),P<0.05.The incidence of rotator cuff re-tear in the observation group(0.00%,0/41)was lower than that in the control group(14.63%,6/41),P<0.05.Conclusions Double-pulley combined with suture bridge technique under arthroscope has better effects in the treatment of rotator cuff injury,which can reduce shoulder pain,accelerate the recovery of shoulder joint function,improve the score of daily living ability,and reduce the risk of rotator cuff re-tear.
