多模态MR与CT小肠造影诊断小肠肿瘤性疾病的价值比较

Clinical value of multimodal MR and CT enterography in the diagnosis of small intestinal neoplastic diseases

目的比较多模态磁共振(MR)与计算机断层扫描(CT)小肠造影诊断小肠肿瘤性疾病的临床价值。方法抽取2020年1月至2022年10月河南省荣康医院收治的经病理证实的小肠肿瘤性疾病患者50例,均接受过小肠造影,包括多模式MR与CT两种。将病理结果作为标准,比较两种检查方法的诊断结果。结果50例小肠肿瘤性疾病患者中,病理结果显示良性肿瘤10例,恶性肿瘤40例。多模态MR与CT诊断效能比较,差异未见统计学意义(P>0.05)。多模态MR、CT小肠造影诊断小肠肿瘤性疾病与病理结果Kappa值分别为0.881、0.861,二者一致性均较高。多模态MR对小肠肿瘤性疾病不同肿瘤部位检出比例与CT比较,差异未见统计学意义(P>0.05)。结论小肠肿瘤性疾病造影检查中,多模态MR及CT在诊断效能、与病理结果一致性、对不同部位肿瘤的检出率方面,均有着相似的结果,临床诊断小肠肿瘤性疾病时可灵活选择这两种方式,且二者联合检查能达到协同互补的作用。

Objective To investigate the clinical value of multimodal magnetic resonance(MR)and computed tomography(CT)enterography in the diagnosis of small intestinal neoplastic diseases.Methods Fifty patients with small intestinal neoplastic diseases diagnosed by pathology in Henan Provincial Rongkang Hospital from January 2020 to October 2022 were selected.All of them underwent enterography,including multimodal MR and CT.The pathological results were taken as the standard to compare the diagnostic results of the two examination methods.Results Among the 50 patients with small intestinal neoplastic diseases,pathological results showed 10 cases benign tumors and 40 cases malignant tumors.There was no significant difference in the diagnostic efficacy between multimodal MR and CT(P>0.05).TheKappa values of small intestinal neoplastic diseases diagnosed by multimodal MR and CT enterography in comparison with pathological results were 0.881 and 0.861,respectively,and the consistency of the two methods were high.There was no significant difference in the detection rate between multimodal MR and CT enterography in diagnosis of small intestinal neoplastic diseases at different tumor sites(P>0.05).Conclusions In the angiography examination of small intestinal neoplastic diseases,multimodal MR and CT have similar results in terms of diagnostic efficiency,consistency with pathological results,and detection rate of different parts of the diagnosis.The two methods can be flexibly selected for clinical diagnosis of small intestinal neoplastic diseases,and the combined examination of the two can achieve synergy and complementarity.
