摘要
目的:探讨早期肺癌患者血清与胸腔积液中细胞角蛋白19片段(CYFRA211)、神经元特异性烯醇化酶(NSE)、鳞状上皮细胞癌抗原(SCC)水平的变化及意义。方法:抽取2019年2月至2022年5月郑州四六〇医院收治的早期肺癌患者86例进行回顾性分析(研究组),依据病理类型分为鳞状细胞癌组(37例)、腺癌组(28例)和小细胞癌组(21例),另抽取同期收治的50例良性肺部病变患者为对照组。比较研究组和对照组、研究组中三个亚组患者的血清与胸腔积液CYFRA211、NSE、SCC水平。结果:研究组患者血清与胸腔积液中CYFRA211、NSE、SCC水平均高于对照组,差异有统计学意义(P<0.05)。鳞状细胞癌组患者血清与胸腔积液CYFRA211、SCC水平均高于腺癌、小细胞癌,且腺癌组高于小细胞癌组(P<0.05);小细胞癌组患者血清与胸腔积液NSE水平高于鳞状细胞癌组、腺癌组,且鳞状细胞癌组低于腺癌组(P<0.05);三组患者胸腔积液CYFRA211、NSE、SCC水平均高于血清,差异有统计学意义(P<0.05)。结论:血清与胸腔积液CYFRA211、NSE、SCC检测对早期肺癌的诊断和分型有一定的价值。
Objective To investigate the changes and the significance of cytokeratin 19 fragment(CYFRA211),neuron-specific enolase(NSE)and squamous cell carcinoma antigen(SCC)in serum and pleural effusion in patients with early lung cancer.Methods A total of 86 patients with early lung cancer treated in Zhengzhou 460 Hospital from February 2019 to May 2022 were selected for the retrospective analysis(study group).According to pathological types,they were divided into squamous cell carcinoma group(37 cases),adenocarcinoma group(28 cases),small cell carcinoma group(21 cases);and 50 patients with benign lung lesions were selected as control group during the same period.The levels of CYFRA211,NSE and SCC in serum and pleural effusion were compared between the study group and control group,and among the three subgroups of the study group.Results The levels of CYFRA211,NSE and SCC in serum and pleural effusion of the study group were higher than those of the control group,and the difference was statistically significant(P<0.05).Levels of CYFRA211 and SCC in serum and pleural effusion in the squamous cell carcinoma group were higher than those in the adenocarcinoma group and small cell carcinoma group,and adenocarcinoma group had higher levels than the small cell carcinoma group(P<0.05).The level of NSE in serum and pleural effusion in the small cell carcinoma group was higher than that in the squamous cell carcinoma group and adenocarcinoma group,moreover,the squamous cell carcinomag group had lower level than the adenocarcinoma group(P<0.05).In the three groups,the levels of CYFRA211,NSE and SCC in pleural effusion were higher than those in serum,and the difference was statistically significant(P<0.05).ConclusionsTThe detection of CYFRA211,NSE,SCC in serum and pleural effusion is valuable for the diagnosis and classification of early lung cancer.
