右美托咪定联合罗哌卡因标准化方案运用在老年股骨近端骨折患者外周阻滞麻醉中的价值

Value of Dexmedetomidine Combined with Ropivacaine in Peripheral Block Anesthesia in Elderly Patients with Proximal Femoral Fractures

目的:探究右美托咪定联合罗哌卡因标准化方案运用在老年股骨近端骨折患者外周阻滞麻醉中的价值。方法:筛选老年股骨近端骨折患者70例,时间范围2021年1月至2022年12月,随机将其分为对照组(罗哌卡因外周阻滞麻醉)和观察组(右美托咪定+罗哌卡因标准化方案外周阻滞麻醉),各35例,判定两组麻醉效果差异。结果:与对照组相比,观察组运动神经、感觉神经阻滞维持时间更长,阻滞起效时间更短,阻滞后5min、阻滞后30min,观察组HR、MAP水平均低于对照组;与对照组相比,术后6h、12h、24h,观察组VAS评分更低,且观察组不良反应发生率更低,差异有统计学意义(P<0.05)。结论:针对老年股骨近端骨折患者,右美托咪定联合罗哌卡因标准化方案阻滞效果理想。

Objective:To investigate the value of dexmedetomidine combined with ropivacaine in peripheral block anesthesia in elderly patients with proximal femoral fracture.Methods:Seventy elderly patients with proximal femoral fracture were screened from January 2021 to December 2022,and randomly divided into control group(Ropivacaine peripheral block anesthesia)and observation group(dexmedetomidine+Ropivacaine standardized program peripheral block anesthesia),with 35 cases in each group.The difference in anesthesia effect between the two groups was determined.Results:Compared with the control group,the maintenance time of motor nerve and sensory nerve block in the observation group was longer,and the onset time of the block was shorter.The HR and MAP levels in the observation group were lower than those in the control group at 5min and 30min after the block.Compared with the control group,the VAS score in the observation group was lower at 6h,12h and 24h after the operation,and the incidence of adverse reactions in the observation group was lower.The difference was statistically significant(P<0.05).Conclusion:For elderly patients with proximal femoral fracture,dexmedetomidine combined with ropivacaine has an idealblockingeffect.
