Practice and Discussion on Social Work of Discharge Planning in Medical Institutions

医务社会工作是医疗服务中的重要组成部分,发展医务社会工作是实施健康中国战略的必然要求,在传递医学人文关怀,解决患者治疗之外的精神、社会性等需求方面发挥着重要作用。社会工作者介入患者出院准备服务,能够满足患者出院后的安置和康复需求,降低其再住院率。在此基础上,提出医疗机构中开展医务社会工作的工作策略:推广出院准备医务社会工作服务;促进“医社”融合,推动学科联动;平等对待患者,尊重患者自决。

Medical social work is an important part of medical services,and the development of medical social work is an inevitable requirement for the implementation of Healthy China strategy.It plays an important role in delivering medical humanistic care and addressing patients'spiritual and social needs beyond treatment.The intervention of social workers in patient discharge planning can meet the settlement and rehabilitation needs of patients after discharge and reduce their readmission rate.On this basis,the work strategies of medical social work in medical institutions are put forward:promoting medical social work services for discharge planning,promoting the integration of"medicine and community"and the linkage of disciplines,treating patients equally and respecting their self-determination.
