八珍汤联合多糖铁复合物改善乳腺癌术后患者肿瘤化疗相关性中度贫血的临床研究

Clinical efficacy of Bazhen Decoction combined with polysaccharide iron complex in improving tumor chemotherapy related moderate anemia in patients with breast cancer after surgery

目的观察八珍汤联合多糖铁复合物改善乳腺癌术后患者肿瘤化疗相关性中度贫血的临床疗效。方法选取2021年5月~2022年5月解放军总医院第一医学中心收治的肿瘤化疗相关性中度贫血的乳腺癌术后住院患者105例,采用随机数表法分为A、B、C 3组,各35例,A组采用重组人促红细胞生成素注射液方案进行治疗,B在A基础上联合口服多糖铁复合物胶囊,C组在B组基础上联合八珍汤口服。观察比较3组患者临床疗效、治疗前后血红蛋白(HGB)、红细胞计数(RBC)、中医证候及KPS评分变化情况。结果3组患者治疗前HGB、RBC组间差异无统计学意义(P>0.05)。治疗8周后,C组RBC显著高于A和B组,差异有统计学意义(P<0.001);C组HGB高于A组,差异有统计学意义(P<0.05);C组HGB高于B组,但差异无统计学意义(P>0.05)。A组治疗有效率为68.57%,B组治疗有效率为85.71%,C组治疗有效率为91.43%,C组有效率与A、B组相比,差异具有统计学意义(P<0.05)。C组患者在倦怠乏力、纳差、失眠多梦、面色少华等中医症状改善明显,与A组、B组相比差异均具有统计学意义(P<0.05)。治疗后B、C组KPS评分较A组相比有改善,差异具有统计学意义(P<0.05),且C组较B组改变更明显,差异有统计学意义(P<0.001)。结论八珍汤联合多糖铁复合物能显著改善肿瘤化疗相关性贫血患者的临床症状、HGB及RBC指标,提升生活质量。

Objective To investigate the clinical efficacy of Bazhen Decoction combined with polysaccharide iron complex in improving tumor chemotherapy related moderate anemia in patients with breast cancer after surgery.Methods 105 patients with tumor chemotherapy related moderate anemia who met the inclusion criteria from May 2021 to May 2022 were selected and randomly divided into three groups:A,B and C(35 cases in each group),group A was treated with recombinant human erythropoietin injection,group B was combined with oral polysaccharide iron complex capsule on the basis of group A,and group C was combined with Bazhen decoction on the basis of group B.Observe and compare the clinical efficacy,changes in hemoglobin,red blood cell count,changes in TCM syndromes and changes in KPS score of the three groups of patients before and after treatment.Results There was no statistically significant difference in blood indicators of the three groups of patients before treatment(P>005),after 8 weeks of treatment,the blood indexes in group C were significantly higher than those in group A and group B(P<0.001),and there was no significant difference in HGB between group B and Group C(P>005).The effective rate of group A was 68.57%,that of group B was 85.71%,and that of group C was 91.43%.The difference between the effective rate of group C and group A was statistically significant(P<0.05),The difference between the effective rate of group C and group B also showed statistically significant.The patients in group C improved significantly in the symptoms of TCM,such as tiredness,anorexia,insomnia and dreaminess,lack of vitality,and the difference was statistically significant compared with group A and group B(P<0.05).After treatment,the KPS score of group B and group C was improved compared with that of group A,and the difference was statistically significant(P<0.05),and the change of group C was more obvious than that of group B,and the difference was statistically significant(P<0.001).Conclusion Bazhen Decoction combined with polysaccharide-iron complex can significantly improve the clinical symptoms,HGB and RBC indexes of tumor chemotherapy-related anemia patients,and improve the quality of life.
