妊娠终止时机对胎膜早破孕产妇分娩方式及母婴结局的影响分析

Analysis of the infuence of pregnancy termination time on delivery mode and maternal and infant outcome of pregnant women with premature rupture of membranes

目的分析妊娠终止时机对胎膜早破孕产妇分娩方式及母婴结局的影响。方法选取30例胎膜早破孕产妇作为研究对象,根据妊娠终止时机的不同分为甲组、乙组、丙组,各10例。甲组孕产妇妊娠终止时机为胎膜早破后的2~12h分娩,乙组孕产妇时机选择为胎膜早破后的13~48h引产分娩,丙组孕产妇时机选择为胎膜早破后的48h以上引产分娩。比较三组孕产妇的分娩方式、孕产妇不良结局情况、新生儿不良结局情况。结果甲组、乙组、丙组的阴道分娩率分别为90.00%、70.00%和60.00%,比较差异有统计学意义(P<0.05)。三组孕产妇不良结局发生率比较,差异有统计学意义(P<0.05)。三组新生儿不良结局发生率比较,差异有统计学意义(P<0.05)。结论对于胎膜早破孕产妇,妊娠终止时机越早越好。在妇产科临床实践中,如果孕产妇剖宫产指征不明显,需要在胎膜早破后2~12h尽早引产,使母婴结局能够得到最大程度的改善。

Objective To analyze the effect of pregnancy termination time on the delivery mode and maternal and infant outcome of pregnant women with premature rupture of membranes.Methods 30 pregnant women with premature rupture of membranes were selected as the research objects and divided into groups A,B and C,with 10 cases each,according to the different timing of pregnancy termination.The timing of pregnancy termination for women in group A was 2 to 12 hours after premature rupture of membranes,the timing for women in group B was 13 to 48 hours after premature rupture of membranes,and the timing for women in group C was 48 hours after premature rupture of membranes.The methods of delivery,adverse outcomes of pregnant women and adverse outcomes of newborns were compared among the three groups.ResultsThe vaginal delivery rates of group A,group B and group C were 90.00%,70.00%and 60.00%,respectively,and the difference was statistically significant(P<0.05).There was statistical significance in the incidence of adverse outcomes among the three groups(P<0.05).There was statistical significance in the incidence of adverse outcomes among the three groups(P<0.05).Conclusion For pregnant women with premature rupture of membranes,the earlier the time of pregnancy termination,the better.In the clinical practice of obstetrics and gynecology,if the indications of cesarean section are not obvious,it is necessary to inducing labor as early as 2 to 12 hours after premature rupture of fetal membrane,so that the outcome of mother and child can be improved to the greatest extent.
