地特胰岛素联合门冬胰岛素治疗妊娠期糖尿病的效果

Effect of insulin detemide combined with insulin aspartate in the treatment of gestational diabetes mellitus

目的探讨地特胰岛素联合门冬胰岛素治疗妊娠期糖尿病(GDM)的效果。方法选取90例GDM患者,按照随机抽签法分为对照组与观察组,每组45例。对照组应用门冬胰岛素治疗,观察组在对照组基础上应用地特胰岛素治疗。比较两组血糖指标、胰岛素用量、血糖达标时间及分娩结局。结果干预前,两组空腹血糖、餐后2h血糖及糖化血红蛋白水平比较,差异无统计学意义(P>0.05);干预后,两组空腹血糖、餐后2h血糖及糖化血红蛋白水平均降低,且观察组低于对照组(P<0.05)。观察组不良妊娠结局发生率为8.89%,低于对照组的31.11%(P<0.05)。观察组胰岛素用量少于对照组,血糖达标时间短于对照组(P<0.05)。结论地特胰岛素联合门冬胰岛素治疗GDM,可降低血糖指标,减少胰岛素用量,治疗时间短,改善分娩结局,临床意义显著。

Objective To investigate the effect of Insulin detemide combined with insulin aspartate in the treatment of gestational diabetes mellitus(GDM).Methods 90 GDM patients were selected and divided into control group and observation group according to random drawing method,with 45 cases in each group.The control group was treated with insulin aspartate,and the observation group was treated with insulin detemide on the basis of control group.Blood glucose index,insulin dosage,blood glucose reaching time and delivery outcome were compared between the two groups.Results Before intervention,there was no significant difference in fasting blood glucose,2 h postprandial blood glucose and glycated hemoglobin levels between the two groups(P>0.05).After intervention,the levels of fasting blood glucose,2 h postprandial blood glucose and HBAlc in both groups were decreased,and the observation group was lower than the control group(P<0.05).The incidence of adverse pregnancy outcome was 8.89%in the observation group,which was lower than 31.11%in the control group(P<0.05).The amount of insulin in the observation group was less than that in the control group,and the time to reach blood glucose standard was shorter than that in the control group(P<0.05).Conclusion Insulin detemide combined with insulin aspartate in the treatment of GDM can reduce blood glucose index,reduce insulin dosage,shorten treatment time,improve delivery outcomes,and have significant clinical significance.
