Risk factors for postoperative pulmonary complications in patients with aortic dissection:A Meta-analysis

目的探讨主动脉夹层(aortic dissection,AD)患者术后肺部并发症的危险因素,为相关人员提供参考。方法检索PubMed、Embase、the Cochrane Library、CNKI等9个数据库,搜集国内外关于AD患者术后肺部并发症的相关研究,检索时限为建库至2023年6月。使用纽卡斯尔-渥太华量表进行质量评价。使用Stata 16.0软件进行数据合成。结果共纳入13篇原始研究,均为病例对照研究,质量较高。Meta分析结果显示,呼吸机使用时间、吸烟和合并糖尿病是AD患者术后发生肺部并发症的危险因素。结论为了预防AD患者术后发生肺部并发症,应在使用呼吸机时及时排出气管内分泌物,患者情况稳定后,尽早拔除气管插管,减少插管留置时间,宣传戒烟,控制好血糖。

Objective To investigate the risk factors for postoperative pulmonary complications in patients with aortic dissection(AD),thus providing clinical references.Methods Domestic and international studies on postoperative pulmonary complications in AD patients published before June 2023 were searched in nine databases,including PubMed,Embase,the Cochrane Library and CNKI,etc.Quality evaluation was performed using the Newcastle-Ottawa scale.Data processing was performed using Stata 16.0 software.Results A total of 13 original studies were included,all of which were high-quality case-control studies.Meta-analysis results showed that duration of ventilator use,smoking,and the comorbidity of diabetes mellitus were risk factors for postoperative pulmonary complications in AD patients.Conclusion To prevent postoperative pulmonary complications in AD patients,endotracheal secretions should be timely drained during the use of a ventilator.After the patient's condition is stabilized,extubate the tracheal intubation as early as possible to shorten the duration of intubation,health education on quitting smoking and blood sugar control are of great significance.
