COOK球囊与前列腺素制剂对妊娠期高血压疾病初产妇足月引产的临床疗效观察

Clinical observation of COOK balloon and prost preparation on term induced labor of primipara with hypertensive diseases during pregnancy

目的研究COOK球囊与前列腺素制剂对妊娠期高血压疾病初产妇足月引产的临床疗效。方法选取60例妊娠期高血压疾病足月引产初产妇(引产前宫颈评分均≤5分),按照随机数字表法分为对照组与试验组,每组30例。对照组采用前列腺素制剂进行引产,试验组采用COOK球囊进行引产。比较两组临床疗效及安全性。结果试验组阴道分娩率83.33%高于对照组的56.67%,平均宫颈成熟度Bishop分、促宫颈成熟率、催产素使用率、人工破膜率均高于对照组(P<0.05);试验组引产期间并发症发生率低于对照组,产后出血量少于对照组(P<0.05);两组初产妇的新生儿结局比较,差异无统计学意义(P>0.05)。结论妊娠期高血压疾病初产妇足月引产通过COOK球囊进行引产,其引产效果优于前列腺素制剂,可减少产后出血量,降低并发症发生率,值得临床推广。

Objective To study the clinical effect of COOK balloon and prost preparation on term induced labor of primipara with hypertensive diseases during pregnancy.Methods 60 parturients with full-term induction of labor with hypertensive diseases during pregnancy(cervical scores≤5 points before induction of labor)were selected and divided into control group and experimental group according to random number table method,with 30 cases in each group.The control group was induced by prost preparation,and the experimental group was induced by COOK balloon.The clinical efficacy and safety of the two groups were compared.Results The vaginal delivery rate of experimental group was 83.33%higher than that of control group(56.67%),the average cervical maturity Bishop score,cervical maturation promotion rate,oxytocin use rate and artificial rupture rate were higher than those of control group(P<0.05).The complication rate of trial group during labor induction was lower than that of control group,and postpartum blood loss was lower than that of control group(P<0.05).There was no significant difference in neonatal outcomes between the two groups(P>0.05).Conclusion Full term induction of labor by COOK balloon is better than prost preparation,which can reduce postpartum blood loss and reduce the incidence of complications,and is worthy of clinical promotion.
