胰岛素泵联合血液透析治疗糖尿病酮症酸中毒的效果 被引量:4

摘要 目的:探讨胰岛素泵联合血液透析治疗糖尿病酮症酸中毒的效果。方法:选取2019年1月-2022年1月在福建医科大学附属闽东医院治疗的85例糖尿病酮症酸中毒患者。根据随机数表法将其分为研究组(43例)和对照组(42例)。两组均给予一般治疗,对照组给予胰岛素泵治疗,研究组在对照组基础上给予血液透析治疗。比较两组临床疗效,治疗前后血糖指标及酸中毒指标、时间指标及不良反应。结果:治疗后,研究组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)均低于对照组,差异有统计学意义(P<0.05)。治疗后,研究组二氧化碳结合力(CO_(2)CP)高于对照组,血酮体(KET)、血乳酸(Lac)均显著低于对照组,差异有统计学意义(P<0.05)。研究组初次酸碱度(pH值)恢复正常时间、初次KET复常时间、初次血糖复常时间均显著早于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:胰岛素泵联合血液透析治疗糖尿病酮症酸中毒能够提升治疗效果,可有效改善患者血糖水平及酸中毒,改善临床症状。 Objective:To investigate the effect of insulin pump combined with hemodialysis in the treatment of diabetes ketoacidosis.Method:A total of 85 patients with diabetic ketoacidosis treated in Mindong Hospital Affiliated to Fujian Medical University from January 2019 to January 2022 were selected.They were divided into study group(43 cases)and control group(42 cases)according to random number table method.Both groups were given general treatment,the control group was given insulin pump treatment,and the study group was given hemodialysis treatment on the basis of the control group.The clinical efficacy,blood glucose index,acidosis index before and after treatment,time index and adverse reaction were compared between the two groups.Result:After treatment,the total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG)and glycosylated hemoglobin(HbA1c)in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the carbon dioxide binding force(CO_(2)CP)of the study group was higher than that of the control group,and the ketone body(KET)and blood lactic acid(Lac)of the study group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).The time of initial pH normalcy,initial KET normalcy and initial blood glucose normalcy in the study group were significantly earlier than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The treatment of diabetic ketoacidosis with insulin pump combined with hemodialysis can improve the therapeutic effect,effectively improve the blood glucose level and acidosis of patients,and improve clinical symptoms.
