阿立哌唑联合丙戊酸钠治疗躁狂症的临床效果及对躁狂症状、社会功能的影响

Clinical Efficacy of Aripiprazole Combined with Sodium Valproate in the Treatment of Mania and Its Effect on Manic Symptoms and Social Function

目的:探讨阿立哌唑联合丙戊酸钠治疗躁狂症的临床效果及对躁狂症状、社会功能的影响。方法:选取2020年3月-2022年9月建瓯市第三医院收治的76例躁狂症患者。根据随机数表法将其分为对照组和观察组,各38例。对照组给予丙戊酸钠片,观察组在对照组基础上给予阿立哌唑片。比较两组临床疗效,治疗前后躁狂症状、社会功能、神经内分泌指标及不良反应。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组贝克-拉范森躁狂量表(Bech-Rafaelsen mania rating scale,BRMS)评分、社会功能缺陷筛选量表(social disability screening schedule,SDSS)评分均低于治疗前,观察组BRMS评分、SDSS评分均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组三碘甲状腺原氨酸(triiodothyronine,T3)与促肾上腺皮质激素(adrenocorticotropic hormone,ACTH)水平均低于治疗前,脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)水平高于治疗前,观察组T3、ACTH水平均低于对照组,BDNF水平高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:对躁狂症患者采取阿立哌唑与丙戊酸钠的联合用药方案,可显著改善患者躁狂症状,改善其社会功能,安全性好。

Objective:To explore the clinical efficacy of Aripiprazole combined with Sodium Valproate in the treatment of mania and its effect on manic symptoms and social function.Method:A total of 76 patients with mania treated in the Jian’ou Third Hospital from March 2020 to September 2022 were selected.They were divided into control group and observation group according to random number table method,with 38 cases in each group.The control group was given Sodium Valproate Tablets,and the observation group was given Aripiprazole Tablets based on the control group.The clinical efficacy,symptoms of mania,social function,neuroendocrine indexes before and after treatment and adverse reactions were compared between two groups.Result:The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of Bech-Rafaelsen mania rating scale(BRMS)and social disability screening schedule(SDSS)in both groups were lower than those before treatment,the scores of BRMS and SDSS in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of triiodothyronine(T3)and adrenocorticotropic hormone(ACTH)in both groups were lower than those before treatment,the level of brain-derived neurotrophic factor(BDNF)was higher than that before treatment,the levels of T3 and ACTH in the observation group were lower than those in the control group,and the level of BDNF was higher than that in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Taking combination medication plan of Aripiprazole and Sodium Valproate for patients with mania can significantly improve the symptoms of mania,improve their social function and have good safety.
