血清hs-CRP、Hcy水平与高血压性视网膜病变的关系研究

摘要 目的:研究血清超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)水平与高血压性视网膜病变(HRP)的关系。方法:选取2021年5月-2022年5月昆明医科大学附属昭通医院收治的105例高血压患者,依据是否合并HRP将其分为HRP组(45例)和非HRP组(60例),另选取同期在本院进行健康体检的30例志愿者作为对照组。测定所有受试者hs-CRP、Hcy水平,采用单因素及logistic回归分析各指标与HRP的关系,并绘制受试者工作特征(ROC)曲线分析hs-CRP、Hcy诊断HRP的效能。结果:HRP组高血压病程较非HRP组长,差异有统计学意义(P<0.05),非HRP组和HRP组收缩压、舒张压均较对照组高,差异有统计学意义(P<0.05);但非HRP组和HRP组收缩压、舒张压比较,差异无统计学意义(P>0.05)。在hs-CRP、Hcy方面,HRP组两项指标水平高于非HRP组,非HRP组高于对照组,差异有统计学意义(P<0.05)。logistic回归分析显示高血压病程、hs-CRP、Hcy均为HRP发生的独立危险因素(P<0.05)。血清hs-CRP诊断HRP的敏感度、特异度分别为62.61%和94.29%,AUC为0.852(0.784,0.904),Hcy诊断HRP的敏感度、特异度则分别为70.43%和91.43%,AUC为0.877(0.814,0.925)。hs-CRP联合Hcy诊断HRP的敏感度、特异度分别为86.09%和88.57%,其AUC为0.940(0.890,0.972)。结论:血清hs-CRP、Hcy在HRP疾病中均呈高表达,两者联合检测诊断HRP的效能更高。 Objective:To study the correlation between serum hypersensitive C-reactive protein(hs-CRP),homocysteine(Hcy)and hypertensive retinopathy(HRP).Method:A total of 105 patients with hypertension admitted to Zhaotong Hospital Affiliated to Kunming Medical University from May 2021 to May 2022 were selected and divided into HRP group(45 cases)and non-HRP group(60 cases)according to whether HRP was combined,30 volunteers who had physical examination in our hospital at the same time were selected as the control group.The levels of hs-CRP and Hcy of all subjects were measured,the relationship between each index and HRP were analyzed by single factor and logistic regression,the receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic efficacy of hs-CRP and Hcy in HRP.Result:The duration of hypertension in HRP group was longer than that in non-HRP group,and the difference was statistically significant(P<0.05).Systolic blood pressure and diastolic blood pressure in non-HRP group and HRP group were higher than those in control group,and the differences were statistically significant(P<0.05).However,there were no significant differences in systolic and diastolic blood pressure between non-HRP group and HRP group(P>0.05).In terms of hs-CRP and Hcy,the indexes in HRP group were higher than that in non-HRP group,the indexes in non-HRP group were higher than that in control group,the differences were statistically significant(P<0.05).logistic regression analysis showed that the duration of hypertension,hs-CRP and Hcy were independent risk factors for HRP(P<0.05).The sensitivity and specificity of serum hs-CRP in the diagnosis of HRP were 62.61%and 94.29%,the AUC was 0.852(0.784,0.904),the sensitivity and specificity of serum Hcy in the diagnosis of HRP were 70.43%and 91.43%,and the AUC was 0.877(0.814,0.925),respectively.The sensitivity and specificity of hs-CRP combined with Hcy in the diagnosis of HRP were 86.09%and 88.57%,respectively,and the AUC was 0.940(0.890,0.972).Conclusion:Serum hs-CRP and Hcy are highly expressed in HRP diseases,and the combined detection has higher diagnostic efficiency.
