失眠认知行为治疗对阻塞性睡眠呼吸暂停低通气综合征患者的影响

Effect of Cognitive Behavioral Therapy for Insomnia on Patients with Obstructive Sleep Apnea Hypopnea Syndrome

目的:探讨失眠认知行为治疗(CBTI)对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的影响。方法:选择2019年5月-2022年5月厦门市仙岳医院睡眠医学中心接诊的80例OSAHS患者为研究对象。采用随机数表法分为对照组和观察组,各40例。对照组采用右佐匹克隆片治疗,在此基础上,观察组采用CBTI,两组的疗程均为6周。比较两组临床疗效、治疗前后的蒙特利尔认知评估量表(MoCA)、简易精神状态检查量表(MMSE)、匹茨堡睡眠质量指数量表(PSQI)、睡眠呼吸暂停低通气指数(AHI)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分。结果:治疗后,观察组临床总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组MoCA评分、MMSE评分均明显高于对照组,PSQI评分、AHI、SAS评分、SDS评分均明显低于对照组,差异有统计学意义(P<0.05)。结论:CBTI应用于OSAHS患者的疗效显著,能改善认知功能和睡眠质量,提高PSQI评分,减少负面情绪的发生。

Objective:To explore the effect of cognitive behavioral therapy for insomnia(CBTI)on obstructive sleep apnea hypopnea syndrome(OSAHS).Method:A total of 80 OSAHS patients admitted to Sleep Medical Center of Xiamen Xianyue Hospital from May 2019 to May 2022 were selected as the study subjects,and they were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with Dexzopiclone Tablets,on this basis,the observation group was treated with CBTI,the course of treatment in both groups was 6 weeks.The clinical efficacy,scores of Montreal cognitive assessment(MoCA),mini mental state examination(MMSE),Pittsburgh sleep quality index(PSQI),sleep apnea-hypopnea index(AHI),self-rating anxiety scale(SAS)and self-rating depression scale(SDS)before and after treatment were compared between the two groups.Result:After treatment,the total clinical effective rate of the observation group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,the MoCA score and MMSE score of the observation group were significantly higher than those of the control group,while the PSQI score,AHI,SAS score,and SDS score were significantly lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion:The application of CBTI in OSAHS patients has significant therapeutic effects,improving cognitive function and sleep quality,improving PSQI scores,and reducing the occurrence of negative emotions.
