基于护士危机意识的预警性管理干预应用于脑卒中偏瘫患者中的效果观察

Observation on the Effect of Early Warning Management Intervention Based on Nurses'Crisis Awareness Applied to Stroke Patients with Hemiplegia

目的:探讨基于护士危机意识的预警性管理干预在脑卒中偏瘫患者中的应用效果。方法:选取2020年1月-2022年6月在江苏省荣军医院接受治疗的116例脑卒中偏瘫患者为研究对象,将其随机分为两组,各58例。对照组实施常规干预,观察组实施基于护士危机意识的预警性管理干预。比较两组下肢深静脉血栓形成(DVT)与下肢肿胀发生情况,肢体运动功能、负性情绪及生活质量。结果:干预1周后,观察组DVT发生率与下肢肿胀发生率均明显低于对照组,差异有统计学意义(P<0.05)。观察组干预1周后上肢评分、下肢评分及总分均明显高于对照组,差异有统计学意义(P<0.05)。观察组干预1周后焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分均明显低于对照组,差异有统计学意义(P<0.05)。观察组干预1周后脑卒中专用生活质量量表(SS-QOL)评分明显高于对照组,差异有统计学意义(P<0.05)。结论:基于护士危机意识的预警性管理干预的应用,可降低脑卒中偏瘫患者发生DVT与下肢肿胀等不良事件,提高其肢体运动功能,同时还能够显著缓解患者的负性情绪,促进其生活质量的改善。

Objective:To explore the application effect of early warning management intervention based on nurses'crisis awareness in stroke patients with hemiplegia.Method:A total of 116 patients with stroke hemiplegia who received treatment in Jiangsu Rongjun Hospital from January 2020 to June 2022 were selected as the study objects and randomly divided into two groups,with 58 patients in each group.The control group was given routine intervention,and the observation group was given early warning management intervention based on nurses'crisis awareness.The incidence of deep vein thrombosis(DVT)and lower limb swelling,limb motor function,negative emotion and quality of life were compared between the two groups.Result:After 1 week of intervention,the incidence of DVT and lower limb swelling in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After 1 week of intervention,the upper limb score,lower limb score and total score of the observation group were significantly higher than those of the control group,and the differences were statistically significant(P<0.05).After 1 week of intervention,self-rating anxiety scale(SAS)scores and self-rating depression scale(SDS)scores in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The stroke specific quality of life scale(SS-QOL)score of the observation group was significantly higher than that of the control group after 1 week of intervention,and the difference was statistically significant(P<0.05).Conclusion:The application of early warning management intervention based on nurses'crisis awareness in stroke patients with hemiplegia can reduce the adverse events such as DVT and lower limb swelling,improve their limb motor function,and significantly relieve patients'negative emotions and promote the improvement of their quality of life.
