老年病房医疗服务管理人员共情疲劳状况分析 被引量:1

Analysis of empathy fatigue among medical service management personnel in geriatric wards

摘要 目的观察老年病房医疗服务管理人员共情疲劳现状,分析相关指标与其疲劳程度的关系。方法纳入2019年1月至2021年1月老年病房医疗服务管理人员124例为研究对象。采用中文版共情疲劳量表对老年病房医疗服务管理人员共情疲劳程度进行评估并分组。设计基线资料填写表,统计老年病房医疗服务管理人员一般基线资料;领悟社会支持量表(PSSS)评估医疗服务管理人员社会支持度,护士职业认同评定量表评估医疗服务管理人员职业自我认同程度,修订版成人心理弹性量表(RSA)评估医疗服务管理人员心理弹性;中文版工作场所心理暴力行为量表(WPVB)评估医疗服务管理人员遭受的心理暴力频率。Kendall’s tau-b相关系数检验分析相关指标与老年病房医疗服务管理人员的相关性。结果124例老年病房医疗服务管理人员中无共情疲劳19例,占15.32%;轻度共情疲劳41例,占33.06%;中度共情疲劳39例,占31.45%;重度共情疲劳25例,占20.16%。4组中正常组PSSS评分、护士职业认同评定量表评分、修订版RSA评分最高,其次为轻度组、中度组,重度组上述各项评分最低(P<0.05);正常组中文版WPVB评分最低,其次为轻度组、中度组,重度组中文版WPVB评分最高(P<0.05)。4组间一般基线资料比较,差异无统计学意义(P>0.05)。Kendall’s tau-b相关系数检验显示,老年病房医疗服务管理人员中文版WPVB评分与其共情疲劳程度呈正相关(r<0,P<0.05),PSSS评分、护士职业认同评定量表评分、修订版RSA评分与其共情疲劳程度呈负相关(r<0,P<0.05)。结论老年病房医疗服务管理人员以轻中度共情疲劳为主,与社会支持度、职业自我认同程度、心理弹性及遭受心理暴力频率有关。 Objective To observe the current situation of empathy fatigue among medical service management personnel in geriatric wards,and to analyze the relationship between related indicators and fatigue levels.Methods A total of 124 medical service management personnel in geriatric wards from January 2019 to January 2021 were included as the research objects.Empathic fatigue scale(Chinese version)was used to evaluate the degree of empathy fatigue among medical service management personnel in geriatric wards and for grouping.The baseline data filling form was designed to collect their general baseline data.The Perceived Social Support Scale(PSSS)was used to evaluate their social support degree,the nurses’professional identity scale was used to evaluate the professional self-identity degree,and the revised adult psychological resilience scale(RSA)was used to evaluate the psychological resilience.The Chinese version of workplace psychological violent behavior scale(WPVB)was applied to assess the frequency of psychological violence suffered by these medical service management personnel.Kendall’s tau-b correlation coefficient test was used to analyze the correlation between related indicators and medical service management personnel in geriatric wards.Results After evaluation,19 out of 124(15.32%)medical service management personnel in geriatric wards had no empathy fatigue,41(33.06%)had mild empathy fatigue,39(31.45%)had moderate empathy fatigue,and 25(20.16%)had severe empathy fatigue.Among the four groups,the normal group had the highest PSSS score,nurses'professional identity scale and revised RSA score,followed by the mild group and the moderate group,and the severe group had the lowest scores(P<0.05).The score of WPVB(Chinese version)in normal group was the lowest,followed by mild group and moderate group,and severe group had the highest score(P<0.05).There was no significant difference in general baseline data among the four groups(P>0.05).Kendall’s tau-b correlation coefficient test showed that WPVB score(Chinese version)of medical service management personnel in geriatric wards was positively correlated with their level of empathy fatigue(r<0,P<0.05),while PSSS score,nurse’professional identity scale,revised RSA score were negatively correlated with their level of empathy fatigue(r<0,P<0.05).Conclusion The medical service management personnel in geriatric wards are mainly characterized by mild to moderate empathy fatigue,which is related to social support,professional self-identity,psychological resilience and the frequency of psychological violence.
