分泌性中耳炎患儿miR-495水平与免疫失衡及炎症状态的相关性

摘要 目的分析分泌性中耳炎患儿血清miR-495水平与免疫失衡及炎症状态的相关性。方法选取2020年3月至2022年3月开封市人民医院收治的分泌性中耳炎合并腺样体肥大患儿53例为观察组,选取同期单纯腺样体肥大患儿40例为对照组。采用RT-qPCR检测血清miR-495水平,采用免疫荧光法检测血液T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)水平,采用酶联免疫吸附试剂盒检测血清中白细胞介素(IL)-8及IL-1β水平,采用Pearson相关系数分析血清miR-495水平与患儿免疫及炎症指标的相关性。结果观察组患儿血清miR-495水平低于对照组(P<0.001)。ROC曲线结果显示血清miR-495水平诊断分泌性中耳炎的曲线下面积为0.969(P<0.001),诊断价值较高。观察组患儿血清中的CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)水平高于对照组,差异有统计学意义(P<0.001)。观察组患儿血清IL-8及IL-1β高于对照组(P<0.001)。经Pearson相关性分析发现观察组患儿血清miR-495水平与T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)水平呈负相关(P<0.05),与炎症因子IL-8及IL-1β水平亦呈负相关(P<0.001)。结论分泌性中耳炎患儿血清miR-495水平异常下调,miR-495可能通过调控机体免疫平衡及炎症反应参与分泌性中耳炎的发生。 Objective To analyze the correlation between serum miR-495 level and immune imbalance and inflammatory state in children with secretory otitis media.Methods From March 2020 to March 2022,53 children with secretory otitis media complicated with adenoidal hypertrophy in Kaifeng People’s Hospital were selected as the observation group,and 40 children with simple adenoidal hypertrophy were selected as the control group.Serum miR-495 level was detected by RT-qPCR,CD3^(+),CD4^(+)and CD8^(+)levels of blood T lymphocyte subsets were detected by immunofluorescence,serum interleukin(IL)-8 and IL-1βlevels were detected by enzyme-linked immunosorbent assay kit,and the correlation between serum miR-495 level and immune and inflammatory indexes of children was analyzed by Pearson correlation coefficient.Results The serum level of miR-495 in the observation group was lower than that in the control group(P<0.001).The receiver operating characteristic(ROC)curve showed that the area under the curve of serum miR-495 level in the diagnosis of secretory otitis media was 0.969(P<0.001),which was of high diagnostic value.The serum levels of CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.001).The serum levels of IL-8 and IL-1βin the observation group were higher than those in the control group(P<0.001).Pearson correlation analysis showed that the serum level of miR-495 was negatively correlated with the levels of T lymphocyte subsets CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+)(P<0.05),and also negatively correlated with the levels of inflammatory factors IL-8 and IL-1β(P<0.001).Conclusion The serum level of miR-495 in children with secretory otitis media is abnormally down-regulated,and miR-495 may participate in the occurrence of secretory otitis media by regulating the immune balance and inflammatory response.
