益生菌联合双环醇治疗非酒精性脂肪肝的效果

Effect of Probiotics Combined with Bicyclic Alcohol on Nonalcoholic Fatty Liver Disease

目的探究益生菌联合双环醇治疗非酒精性脂肪肝(NAFLD)的效果及对脂肪肝超声评分、肝纤维化、炎症状态的影响。方法选取2020年7月至2022年7月新乡市传染病医院156例NAFLD患者,随机分为两组,各78例。两组均接受常规护肝治疗,于此基础上,给予对照组双环醇治疗,给予研究组双歧杆菌三联活菌联合双环醇治疗,均治疗3个月。比较两组治疗效果、肝功能指标[总胆红素(T-Bil)、谷草转氨酶(GOT)、谷丙转氨酶(GPT)、谷氨酰转移酶(GGT)]、肠道菌群、脂肪肝超声评分、肝纤维化指标[透明质酸(HA)、Ⅲ型胶原前肽(PCⅢ)]、炎症指标[白介素-6(IL-6)、正五聚体蛋白3(PTX3)、核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)]、不良反应。结果研究组总有效率(94.87%)高于对照组(84.62%)(P<0.05);研究组治疗1个月、3个月后血清T-Bil、GOT、GPT、GGT水平低于对照组(P<0.05);研究组治疗1个月、3个月后双歧杆菌、乳杆菌数量高于对照组,肠杆菌、葡萄球菌数量低于对照组(P<0.05);研究组治疗1个月、3个月后脂肪肝超声评分、血清HA、PCⅢ水平低于对照组(P<0.05);研究组治疗1个月、3个月后血清IL-6、PTX3、NLRP3水平低于对照组(P<0.05);研究组不良反应发生率(5.13%)与对照组(10.26%)比较,差异无统计学意义(P>0.05)。结论益生菌联合双环醇治疗NAFLD的效果显著,且能改善脂肪肝超声评分、肝纤维化,减轻机体炎症状态。

Objective To investigate the efficacy of probiotics combined with bicyclic alcohol in the treatment of nonalcoholic fatty liver disease(NAFLD)and the effects on ultrasonic scores,fibrosis and inflammation of fatty liver.Methods A total of 156 patients with NAFLD from Xinxiang Hospital of Infectious Diseases from July 2020 to July 2022 were randomly divided into two groups,with 78 patients in each group.Both groups received conventional liver protection treatment,and on this basis,the control group was given bicyclic alcohol treatment,and the study group was given bifidobacterium triple live bacteria combined with bicyclic alcohol treatment,both of which were treated for 3 months.The therapeutic effect,liver function indexes[total bilirubin(T-Bil),glutamic-oxaloacetic transaminase(GOT),glutamic-pyruvic transaminase(GPT),glutamyltransferase(GGT)],intestinal microbiota,fatty liver ultrasound score,liver fibrosis indexes[hyaluronic acid(HA),procollagen typeⅢ(PCⅢ)]and inflammatory status indexes[interleukin-6(IL-6),pentraxin 3(PTX3),nucleotide-bound oligomerized domain-like receptor protein 3(NLRP3)],adverse reactions were compared between the two groups.Results The total effective rate of the study group(94.87%)was higher than that of the control group(84.62%)(P<0.05).The levels of serum T-Bil,GOT,GPT and GGT were lower than those of the control group after 1 month and 3 months of treatment in the study group(P<0.05).The numbers of Bifidobacteria and Lactobacillus were higher than those of the control group and the numbers of Enterobacteriaceae and Staphylococcus were lower than those of the control group after 1 month and 3 months of treatment in the study group(P<0.05).Ultrasound scores of fatty liver,serum HA,PCⅢlevels in the study group were lower than those in the control group after 1 month and 3 months of treatment(P<0.05).Serum IL-6,PTX3,NLRP3 levels in the study group were lower than those in the control group after 1 month and 3 months of treatment(P<0.05).There was no statistical difference in the incidence of adverse reactions between the study group(5.13%)and the control group(10.26%)(P>0.05).Conclusion Probiotics combined with bicyclic alcohol have significant curative effect in the treatment of NAFLD,and can improve the ultrasonic score of fatty liver,liver fibrosis,and reduce the inflammatory state of the body.
