集中带量采购政策对某院质子泵抑制剂使用情况的影响

Impact of National Centralized Drug Procurement Policy on the Use of Proton Pump Inhibitors in a Hospital

目的分析集中带量采购(简称集采)政策对某三级甲等综合医院质子泵抑制剂(PPI)使用的影响。方法回顾性分析集采后(2021年10月1日至2022年3月31日)及集采前(2019年10月1日至2020年3月31日)PPI使用量、使用金额,分别从用药频度(DDDs)、限定日费用(DDC)、日费用比、使用金额等方面对集采政策实施前后PPI使用情况进行分析。结果集采政策实施后,PPI使用总量由12964320 mg降至12404770 mg,降幅为4.32%;使用总金额由601.29万元降至247.46万元,降幅为58.85%;DDDs由506420.83降至483014.00,降幅为4.62%;多数PPI的DDC呈减少趋势,日费用比小于1.0;DDC共节省380.52万元;中选PPI使用量、DDDs、使用金额占比分别为80.39%,81.31%,56.08%。结论集采政策的实施能减轻患者负担,减少医保资金支出,降低医院运营成本。

Objective To analyze the impact of National Centralized Drug Procurement(NCDP)policy on the use of proton pump inhibitors(PPIs)in a tertiary A general hospital,and to provide a reference for pharmaceutical care and hospital pharmacy management.Methods The usage amount and consumption sum of PPIs after(from October 1,2021 to March 31,2022)and before(from October 1,2019 to March 31,2020)the implementation of the NCDP policy.The use of PPIs before and after the implementation of the NCDP policy was analyzed from the defined daily dose system(DDDs),defined daily costs(DDC),daily cost ratio,and consumption sum.Results After the implementation of the NCDP policy,the total usage amount of PPIs decreased from 12964320 mg to 12404770 mg,with a decrease of 4.32%;the total consumption sum decreased from CNY 6.0129 million to CNY 2.4746 million,with a decrease of 58.85%;the DDDs decreased from 506420.83 to 483014.00,with a decrease of 4.62%;the DDC of most PPIs showed a decreasing trend,with a daily cost ratio of less than 1.0;the DDC saved a total of CNY 3.8052 million;the proportion of usage amount,DDDs,and consumption sum of bid-winning PPIs were 80.39%,81.31%,and 56.08%,respectively.Conclusion The implementation of the NCDP policy can reduce the burden of patients,lessen medical insurance expenditure,and reduce hospital operating costs.
