PCT联合LAC用于急性心肌梗死并心源性休克诊断中的临床价值

Clinical Value of PCT Combined with LAC in the Diagnosis of Acute Myocardial Infarction with Cardiogenic Shock

目的分析降钙素原(procalcitonin,PCT)和乳酸(lactate,LAC)对急性心肌梗死并心源性休克的诊断价值。方法回顾性分析2021年2月—2023年2月南通大学附属启东医院检验科90例急性心肌梗死患者的临床资料,根据患者是否合并心源性休克分成合并组(n=49)和未合并组(n=41),对比两组患者PCT和LAC水平,经受试者工作特征(receiver operating characteristic,ROC)曲线分析单项指标和联合指标对疾病的诊断价值。结果合并组患者PCT和LAC水平为(0.18±0.05)ng/mL和(2.78±0.72)mmol/L,均高于未合并组,差异有统计学意义(t=8.977、7.526,P<0.05);ROC曲线显示,PCT联合LAC诊断急性心肌梗死并心源性休克的灵敏度和特异度均为100.00%,高于PCT的75.51%和95.12%,LAC的79.59%和85.37%。结论急性心肌梗死患者合并心源性休克时,可导致PCT、LAC水平上升,联合检测两项指标,可对疾病予以准确诊断。

Objective To analyze the diagnostic value of procalcitonin(PCT) and lactate(LAC) in acute myocardial infarction complicated with cardiogenic shock.Methods Retrospectively analyed was conducted on the clinical data of 90 patients with acute myocardial infarction in the Laboratory of Qidong Hospital Affiliated to Nantong University from February 2021 to February 2023.The patients were divided into a combined group(n=49) and an non combined group(n=41) based on whether they had concomitant cardiogenic shock.Compared the levels of PCT and LAC between two groups of patients,and analyzed the diagnostic value of individual and combined indicators on the receiver operating characteristic(ROC) curve.Results The levels of PCT and LAC in the combined group were(0.18±0.05) ng/mL and(2.78±0.72) mmol/L,both higher than those in the non combined group,the difference was statistically significant(t=8.977,7.526,P<0.05).The ROC curve shows that the sensitivity and specificity of PCT combined with LAC in diagnosing acute myocardial infarction with cardiogenic shock are both 100.00%,higher than 75.51% and 95.12% of PCT,and 79.59% and 85.37% of LAC.Conclusion Patients with acute myocardial infarction complicated with cardiogenic shock can lead to the increase of PCT and LAC levels.The combined detection of these two indexes can accurately diagnose the disease.
