MRI在类风湿膝关节炎患者中的诊断价值

Diagnostic Value of MRI in Patients with Rheumatoid Knee Arthritis

目的 探讨对类风湿膝关节炎患者给予MRI诊断后获得的临床效果。方法 选取2020年3月—2022年5月聊城市东昌府人民医院的40例疑似类风湿膝关节炎患者作为研究对象,分别对其展开X线片检查以及MRI检查,将关节镜检查结果作为诊断金标准。比较两种诊断方式的诊断效能以及分级情况。结果 MRI检查疑似类风湿膝关节炎患者诊断特异度(93.33%)、敏感度(92.00%)、准确度(92.50%)同X线诊断特异度(33.33%)、敏感度(20.00%)、准确度(25.00%)比较,差异有统计学意义(X^(2)=7.111,16.055,25.037,P<0.05);经关节镜检查后,25例患者中,Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级患者例数分别为1例、9例、9例以及6例;MRI在Ⅱ级诊断方面同X线片比较,差异有统计学意义(P<0.05),其他分级比较则差异无统计学意义(P>0.05)。结论 同X线片比较,有效运用MRI诊断方式,可以显著提高诊断敏感度、特异度以及准确度,并且在疾病分级诊断期间,MRI凸显了应用价值。

Objective To explore the clinical effect of MRI diagnosis on patients with rheumatoid knee arthritis.Methods Forty patients with suspected rheumatoid knee arthritis in Dongchangfu People's Hospital of Liaocheng City from March 2020 to May 2022 were selected as the study objects.X-ray examination and MRI examination were performed on them respectively,and the results of arthroscopy were taken as the diagnostic gold standard.The diagnostic efficiency and classification of the two diagnostic methods were compared.Results The diagnostic specificity(93.33%),sensitivity(92.00%) and accuracy(92.50%) of MRI patients suspected of rheumatoid knee arthritis were compared with that of X-ray diagnosis(33.33%),sensitivity(20.00%) and accuracy(25.00%),and the difference was statistically significant(χ^(2)=7.111,16.055,25.037,P<0.05);After arthroscopy,among the 25 patients,the number of patients belonging to grade Ⅰ,grade Ⅱ,grade Ⅲ and grade Ⅳ were 1,9,9 and 6,respectively.The difference between MRI and X-ray film in the diagnosis of grade Ⅱ was statistically significant(P<0.05),while there was no statistically significant difference in other grades(P>0.05).Conclusion Compared with X-ray film,the effective use of MRI diagnosis can significantly improve the diagnostic sensitivity,specificity and accuracy,and MRI highlights the application value during the classification of disease diagnosis.
