甲状腺微小癌应用彩色多普勒超声诊断的效果

Diagnosis Effect of Color Doppler Ultrasonography in Thyroid Microcarcinoma

目的 探讨在甲状腺微小癌的临床诊断中彩色多普勒超声(彩超)的临床应用效果与价值。方法 随机抽取2020年6月—2022年7月山东省菏泽市牡丹人民医院接诊的疑似甲状腺微小癌患者112例,所有受检者均需开展常规二维超声及彩超检查,将手术病理诊断结果设定为金标准,对比分析不同超声检查方式对甲状腺微小癌的诊断效能,同时总结对比甲状腺良性结节与甲状腺微小癌的彩超声像特征。结果 病理诊断结果显示在112例疑似患者中甲状腺微小癌阳性43例,阴性69例。彩超及常规二维超声对甲状腺微小癌的诊断效能分别为:灵敏度95.35%、72.09%,准确性94.64%、79.46%,特异度94.20%、84.06%,差异有统计学意义(χ^(2)=6.750、10.240、4.000,P<0.05)。甲状腺微小癌患者纵横比≥1、微小钙化、血流丰富、边界模糊及低回声等特征率显著高于甲状腺良性结节患者,差异有统计学意义(P<0.05)。结论 临床应用彩超诊断甲状腺微小癌效果确切,敏感度及特异度均较高,且能够有效鉴别良恶性病变,同时还具有操作简单和可重复检查的优势。

Objective To explore the clinical application effect and value of color Doppler ultrasound(color Doppler ultrasound) in the clinical diagnosis of thyroid microcarcinoma.Methods A total of 112 suspected patients with thyroid microcarcinoma from June 2020 to July 2022 in Mudan People's Hospital of Heze City,Shandong Province were randomly selected.All the patients were required to undergo routine twodimensional ultrasound and color ultrasound examination,and the surgical pathological diagnosis results were set as the gold standard,and the diagnostic efficiency of different ultrasonic examination methods for thyroid microcarcinoma was compared and analyzed.At the same time,the color ultrasonic characteristics of benign thyroid nodules and thyroid microcarcinoma were summarized and compared.Results The pathological diagnosis showed that 43 of the 112 suspected patients were positive for thyroid microcarcinoma and 69 were negative.The diagnostic efficacy of color ultrasound and conventional two-dimensional ultrasound for thyroid microcarcinoma was as follows:sensitivity 95.35%,72.09%,accuracy 94.64%,79.46%,specificity 94.20%,84.06%,respectively,the difference was statistically significant(χ^(2)=6.750,10.240,4.000,P<0.05).Aspect ratio ≥1,minimal calcification,rich blood flow,blurred boundary and low echo in patients with thyroid microcarcinoma were significantly higher than those in patients with benign thyroid nodules,the difference was statistically significant(P<0.05).Conclusion The clinical application of color Doppler ultrasound in the diagnosis of thyroid microcarcinoma is effective,with high sensitivity and specificity,and can effectively distinguish benign and malignant lesions.It also has the advantages of simple operation and repeatable inspection.
