PVP联合高粘度骨水泥治疗老年骨质疏松性椎体压缩骨折的效果分析

摘要 目的 研究经皮穿刺椎体成形术(percutaneous vertebro plasty,PVP)联合高粘度骨水泥治疗老年骨质疏松性椎体压缩骨折的效果。方法 选取2020年7月—2023年1月徐州医学院附属连云港市东方医院诊治的137例老年骨质疏松性椎体压缩骨折患者为研究对象,经随机数表法分为PVP组(n=69)、经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)组(n=68),PKP组采用PKP治疗,PVP组采用PVP联合高粘度骨水泥治疗,期间收集并分析患者的手术指标、恢复指标、疼痛感、Oswestry功能障碍指数量表(Oswestry Disability Index,ODI)评分、术后并发症。结果 PVP组骨水泥注射量、手术时间、X线照射时间均低于PKP组,差异有统计学意义(P<0.05)。术后,PVP组Cobb角、椎体高度压缩比值等指标与PKP组相比,差异无统计学意义(P>0.05)。术后1个月、2个月、3个月,PVP组的疼痛感低于PKP组,差异有统计学意义(P<0.05),两组ODI评分相比,差异无统计学意义(P>0.05)。PVP组术后并发症总发生率1.45%低于PKP组11.76%,差异有统计学意义(χ^(2)=4.375,P<0.05)。结论 在针对老年骨质疏松性椎体压缩骨折患者进行治疗时,选择PVP联合高粘度骨水泥治疗能够有效缩短手术操作时间、减少患者X线照射时间,患者术后椎体高度恢复程度以及身体功能良好,有效降低患者疼痛感,并具有更高的安全性,建议在实际的临床治疗中借鉴。 Objective To investigate the efficacy of percutaneous vertebro plasty(PVP)combined with high viscosity bone cement in the treatment of osteoporotic vertebral compression fractures in elderly patients.Methods A total of 137 elderly patients with osteoporotic vertebral compression fractures who were treated in Lianyungang Oriental Hospital Affiliated to Xuzhou Medical College from July 2020 to January 2023 were selected as the research objects.They were divided into PVP group(n=69)and percutaneous kyphoplasty(PKP)group(n=68)by random number table method.The PKP group was treated with PKP,and the PVP group was treated with PVP combined with high vis⁃cosity bone cement.The surgical indicators,recovery indicators,pain,Oswestry Disability Index(ODI)score,and postoperative complications were collected and analyzed.Results The bone cement injection volume,operation time and X-ray irradiation time in PVP group were lower than those in PKP group,and the differences were statistically significant(P<0.05).After operation,there was no statistically significant dif⁃ference in Cobb angle and vertebral height compression ratio between PVP group and PKP group(P>0.05).At 1 month,2 months and 3 months after operation,the pain of PVP group was lower than that of PKP group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in ODI score between the two groups(P>0.05).The total incidence of postoperative complica⁃tions in the PVP group was 1.45%,lower than that in the PKP group(11.76%),and the difference was statistically significant(χ^(2)=4.375,P<0.05).Conclusion In the treatment of elderly patients with osteoporotic vertebral compression fracture,PVP combined with high viscosity bone cement can effectively shorten the operation time,reduce the time of X-ray irradiation of patients,the degree of postoperative vertebral recov⁃ery of patients and good physical function,effectively reduce the pain of patients,and have higher safety,which is recommended for reference in actual clinical treatment.
