复方脑肽节苷脂+多奈哌齐治疗脑梗死后认知障碍的效果探讨

Exploring the Effect of Compound Porcine Cerebroside and Ganglioside+Donepezil in Treating Cognitive Impairment after Cerebral Infarction

目的 探讨复方脑肽节苷脂联合多奈哌齐在脑梗死后认知障碍中的治疗效果。方法 随机选取2020年1月—2022年3月如东县中医院收治的90例脑梗死后存在认知障碍患者为研究对象,根据随机数表法分成对照组(45例)和观察组(45例)。对照组患者接受多奈哌齐治疗,观察组患者则接受多奈哌齐+复方脑肽节苷脂治疗。对比两组的神经功能指标分数、相关指标在治疗前后变化以及用药后不良反应情况。结果 治疗前,两组神经功能、认知评估及精神状态评分比较,差异无统计学意义(P>0.05)。治疗后,观察组神经功能评分低于对照组,认知评估及精神状态评分高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组脑源性神经细胞营养因子、神经元特异性烯醇化酶、同型半胱氨酸水平均优于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率(6.67%)与对照组的13.33%比较,差异无统计学意义(χ^(2)=0.494,P>0.05)。结论 复方脑肽节苷脂与多奈哌齐联合应用可明显改善脑梗死患者认知障碍,改善患者相关指标水平。

Objective To explore the therapeutic effect of compound porcine cerebroside and ganglioside combined with donepezil in the treatment of cognitive impairment after cerebral infarction.Methods 90 patients with cognitive impairment after cerebral infarction admitted to Rudong County Traditional Chinese Medicine Hospital from January2020 to March 2022 were randomly selected as the research subjects.According to the random number table,they were divided into a control group (45 cases) and an observation group (45 cases).The control group patients received donepezil treatment,while the observation group patients received donepezil+compound brain peptide ganglioside treatment.Compared the scores of neurological function indicators,the numerical changes of indicators before and after treatment,and the adverse reactions after medication between the two groups.Results Before treatment,there was no statistically significant difference in the scores of NIHSS,MoCA,and MMSE between the two groups (P<0.05).After treatment,the NIHSS score of the observation group was lower than that of the control group,while the MoCA and MMSE scores were higher than those of the control group,the difference was statistically significant (P<0.05).After treatment,the levels of BDNF,NSE,and Hcy in the observation group were better than those in the control group,the difference was statistically significant (P<0.05).The incidence of adverse reactions in the observation group was 6.67%compared to 13.33%in the control group,and there was no statistically significant difference (χ^(2)=0.494,P<0.05).Conclusion The combination of compound brain peptide gangliosides and donepezil can significantly improve cognitive impairment in patients with cerebral infarction,reduce NSE and Hcy levels,and increase BDNF levels.
