糖皮质激素联合丙种球蛋白对小儿重症肺炎临床症状及血清炎性因子水平的影响

Effect of Glucocorticoid Combined with Gamma Globulin on Clinical Symptoms and Serum Inflammatory Factor Levels in Children with Severe Pneumonia

目的 探察小儿重症肺炎采用糖皮质激素联合丙种球蛋白治疗对减轻临床症状及改善血清炎性因子水平的影响。方法 方便选取2021年12月—2022年12月惠州市中心人民医院、惠州市第一人民医院儿科所收治的重症肺炎患儿94例,按不同治疗方法分组,每组47例,行糖皮质激素治疗的为对照组,在此基础上联合丙种球蛋白治疗的为治疗组,比较两组临床症状改善情况、治疗前后血清炎性因子水平变化情况、临床疗效。结果 治疗组平均退热时间、啰音消失时间与阴影吸收时间均明显短于对照组,差异有统计学意义(P<0.05);治疗后治疗组各项血清炎性因子水平改善情况优于对照组,差异有统计学意义(P<0.05);治疗组临床疗效率为95.74%,显著高于对照组的78.72%,差异有统计学意义(χ^(2)=6.114,P<0.05)。结论 小儿重症肺炎采用糖皮质激素联合丙种球蛋白治疗方法,可加快小儿肺部阴影吸收、啰音消失及退热速度,并尽快恢复血清炎性因子水平在正常范围内,提高机体免疫力,维护其机体健康,疗效确切。

Objective To investigate the effect of glucocorticoid combined with gamma globulin on reducing clinical symptoms and improving the level of serum inflammatory factors in children with severe pneumonia.Methods A total of 94 children with severe pneumonia admitted to the Department of Pediatrics of Huizhou Central People's Hospital and the First People's Hospital from December 2021 to December 2022 were conveniently selected and divided into 47 children in each group according to different treatment methods.Glucocorticoid treatment was used as the control group,and gamma globulin combined treatment was used as the treatment group.The improvement of clinical symptoms,the changes of serum inflammatory factors before and after treatment,and the clinical efficacy of the two groups were compared.Results The average fever time,rales disappearance time and shadow absorption time in the treatment group were significantly shorter than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the improvement of serum inflammatory factors in the treatment group was better than that in the control group,and the difference was statistically significant(P<0.05).The clinical efficacy rate of the treatment group was 95.74%,which was significantly higher than that of the control group(78.72%),and the difference was statistically significant(χ^(2)=6.114,P<0.05).Conclusion The treatment of glucocorticoid combined with gamma globulin in children with severe pneumonia can speed up the absorption of lung shadow,disappearance of rales and fever reduction,and restore the level of serum inflammatory factors within the normal range as soon as possible,improve the immunity of the body,maintain the health of the body,the curative effect is definite.
