基于Neuman模式下个体化护理模式在持续血液净化重症中的效果

Effect of Neuman model-based individualized nursing intervention in continuous blood purification S of severedisease

目的 探讨Neuman模式下个体化护理模式在持续血液净化重症中的效果。方法 选取我院2018年3月至2019年3月收治的42例接受持续血液净化重症患者设为对照组,实施常规护理模式,选取我院2019年4月至2020年4月收治的43例接受持续血液净化重症患者设为观察组,在常规模式的基础上实施Neuman模式下个体化护理干预。对比两组患者护理干预前后的生活水平、心理状态、并发症发生率。结果 干预后,两组患者SF-36评分同组内干预前比较,SF-36评分均有所改善,且观察组SF-36评分改善更加明显(P<0.05);两组患者干预后SAS、SDS评分同组内干预前比较均有所改善,且观察组改善更加明显(P<0.05);观察组患者低血压1例、出血1例,并发症总发生率为4.64%,对照组低血压发生4例、出血2例、肌肉痉挛1例、感染2例,并发症总发生率为21.42%,观察组并发症发生率同对照组比较,低于对照组(P<0.05)。结论 持续血液净化重症患者实施Neuman干预模式下个体化护理能有效提高生活质量、改善负面的情绪、且减少并发症的发生率,值得临床推广使用。

Objective To investigate the effect of individualized nursing intervention under Neuman model in continuous blood purification severe disease.Methods A total of 42 critically ill patients who received continuous blood purification in our hospital from March 2018 to March 2019 were selected as the control group and given conventional nursing model,while 43 critically ill patients who received continuous blood purification in our hospital from April 2019 to April 2020 were selected as the observation group and given individualized nursing intervention under Neuman model on the basis of coventional model.The quality of life,psychological status and the incidence of complications were compared between the two groups before and after nursing intervention.Results After the intervention,the SF-36 scores of the two groups were improved compared with those before the intervention,and the improvement of SF-36 scores in the observation group was more obvious(P<0.05);After the intervention,the scores of SAS and SDS in the two groups were improved compared with those before the intervention,and the improvement in the observation group was more obvious(P<0.05);In the observation group,there were 1 case of hypotension and 1 case of bleeding,and the total incidence of complications was 4.64%.In the control group,there were 4 cases of hypotension,2 cases of bleeding,1 case of muscle spasm and 2 cases of infection,and the total incidence of complications was 21.42%.Compared with the control group,the incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Individualized nursing intervention under Neuman intervention mode can effectively improve the quality of life in critically ill patients with continuous blood purification,relieve their anxiety,depression and other adverse emotions,reduce the incidence of complications.It is worthy of clinical application.
