颈痛汤内服配合整脊推拿治疗风寒湿痹型神经根型颈椎病

Clinical study on the treatment of cervical spondylotic radiculopathy with wind-cold-dampness arthralgia type by oral Jingtong Tang combined with chiropractic massage

目的探讨颈痛汤内服配合整脊推拿治疗风寒湿痹型神经根型颈椎病的临床疗效。方法按照随机数表法将92例风寒湿痹型神经根型颈椎病患者分成对照组和观察组,各46例。对照组采用颈椎牵引联合盐酸乙哌立松片内服治疗,观察组在对照组治疗基础上联合颈痛汤内服、整脊推拿治疗。观察2组治疗前后中医证候积分、视觉模拟评分法(VAS)评分、颈椎功能障碍指数(NDI)评分、生活质量评分(SF-36)、颈椎活动度、血液流变学指标水平变化,并比较2组治疗总有效率及安全性。结果观察组治疗后各项中医证候积分、VAS评分、NDI评分、各项血液流变学指标水平均低于对照组(P<0.05),SF-36评分、颈椎活动度及治疗总有效率均高于对照组(P<0.05);观察组总有效率(95.65%)明显高于对照组(82.61%);2组治疗期间均无严重不良反应发生。结论颈痛汤内服联合整脊推拿治疗风寒湿痹型神经根型颈椎病疗效确切,能够有效缓解上肢和颈肩部疼痛等不适症状,调节血液流变学指标,提高颈椎活动功能,改善患者生活质量。

Objective To explore the clinical efficacy of oral administration of Jingtong Tang combined with chiropractic massage in the treatment of cervical spondylotic radiculopathy with wind-cold-dampness arthralgia type.Methods 92 patients with cervical spondylotic radiculopathy with wind-cold-dampness arthralgia type were divided into the control group and the observation group according to random number table method,with 46 cases in each group.The control group was treated with cervical traction combined with oral administration of Eperisone hydrochloride tablets,and the observation group was treated with combination of oral administration of Jingtong Tang and chiropractic massage on the basis of the treatment of the control group.The changes of traditional Chinese medicine(TCM)syndrome score,visual analogue scale(VAS)score,neck disability index(NDI)score,the MOS item short from health survey(SF-36),cervical mobility and hemorheology index level before and after treatment in the two groups were observed.The total effective rate and safety of the two groups were compared.Results After treatment,in the observation group the TCM syndrome scores,VAS scores,NDI scores,and hemorheology indexes levels were lower than those in the control group(P<0.05),the SF-36 scores and cervical mobility were all higher than those in the control group(P<0.05);The total effective rate in the observation group(95.65%)was significantly higher than that in the control group(82.61%)(P<0.05);No serious adverse reactions occurred during the treatment of both groups.Conclusion Oral administration of Jingtong Tang combined with chiropractic massage has a definite therapeutic effect on the treatment of cervical spondylotic radiculopathy with wind-cold-dampness arthralgia type,which can effectively relieve discomfort symptoms such as upper limb pain and neck shoulder pain,adjust hemorheology indicators,improve cervical spine activity,and improve the quality of life of patients.
