益气脱敏汤联合氯雷他定片治疗小儿变应性鼻炎肺气虚寒证30例临床观察

Clinical observation on 30 cases of Yiqi Tuomin Tang(益气脱敏汤)combined with loratadine in the treatment of pediatric allergic rhinitis with lung qi deficiency and cold syndrome

目的 观察益气脱敏汤联合氯雷他定片治疗小儿变应性鼻炎肺气虚寒证的临床疗效。方法 选取2019年10月至2021年10月在永康市第一人民医院耳鼻喉科门诊就诊的变应性鼻炎肺气虚寒证患儿60例,采用随机数字表法分为治疗组和对照组,各30例。对照组予氯雷他定片口服,治疗组在对照组治疗方法的基础上予益气脱敏汤治疗。2组均1周为1个疗程,连续治疗3个疗程后统计疗效。结果 对照组总有效率为76.67%(23/30),治疗组为96.67%(29/30),2组比较,差异有统计学意义(P<0.05)。治疗前2组中医主症、次症积分及总积分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组上述积分均显著下降,与同组治疗前比较,差异均有统计学意义(P<0.05),且治疗组下降更显著(P<0.05)。治疗前2组鼻腔阻力和鼻腔通气面积比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组鼻腔阻力显著下降,鼻腔通气面积显著增大,与同组治疗前比较,差异均有统计学意义(P<0.05),且治疗组下降或增大更显著(P<0.05)。治疗前2组血清特异性免疫球蛋白E(sIgE)和白细胞介素-4(IL-4)水平比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组血清sIgE和IL-4水平均显著下降,与同组治疗前比较,差异有统计学意义(P<0.05),且治疗组下降更显著(P<0.05)。结论 益气脱敏汤联合氯雷他定片治疗小儿变应性鼻炎肺气虚寒证疗效显著,能明显缓解患儿的临床症状,改善鼻腔功能和机体免疫力,减轻炎症反应,值得临床推广应用。

Objective To observe the clinical efficacy of Yiqi Tuomin Tang(益气脱敏汤)combined with loratadine in the treatment of pediatric allergic rhinitis with lung qi deficiency and cold syndrome.Methods Sixty children with allergic rhinitis and lung qi deficiency and cold syndrome who visited the Department of Otorhinolaryngology at the First People's Hospital in Yongkang City from October 2019 to October 2021 were selected and randomly divided into a treatment group and a control group,with 30 cases in each group.The control group was treated with loratadine tablets,and the treatment group was treated with Yiqi Tuomin Tang on the basis of the treatment of the control group.One week was a course,and the therapeutic effect was evaluated after three consecutive courses of treatment.Results The total effective rate of the control group was 76.67%(23/30),while that of the treatment group was 96.67%(29/30),with a statistically significant difference between the two groups(P<0.05).Before treatment,there was no statistically significant difference in the main symptoms,secondary symptoms,and total scores between the two groups(P<0.05),which were comparable;after treatment,the above scores in both groups significantly decreased,and the difference compared with the same group before treatment was statistically significant(P<0.05),and the decrease in the treatment group was more significant(P<0.05).There was no statistically significant difference in nasal resistance and nasal ventilation area between the two groups before treatment(P>0.05),which were comparable;after treatment,the nasal resistance in both groups significantly decreased,and the nasal ventilation area significantly increased,and the difference compared with the same group before treatment was statistically significant(P<0.05),and the decrease or increase in the treatment group was more significant(P<0.05).There was no statistically significant difference in the levels of serum specific immunoglobulin E(sIgE)and interleukin-4(IL-4)between the two groups before treatment(P>0.05),which were comparable;after treatment,the levels of serum sIgE and IL-4 in both groups significantly decreased,and the difference compared with the same group before treatment was statistically significant(P<0.05),and the decrease in the treatment group was more significant(P<0.05).Conclusion Yiqi Tuomin Tang combined with loratadine has significant efficacy in the treatment of pediatric allergic rhinitis with lung qi deficiency and cold syndrome.It can significantly relieve the clinical symptoms of children,improve nasal function and immune function,and reduce inflammatory reactions,and is worthy of clinical promotion and application.
