丹栀逍遥散加减联合火针治疗肝郁化火证神经性皮炎临床观察

Efficacy observation of modified Danzhi Xiaoyao powder and fire needling on neurodermatitis with syndrome of liver depression transforming into fire

目的:观察丹栀逍遥散加减联合火针治疗肝郁化火证神经性皮炎的临床疗效和对患者负性情绪的影响。方法:将60例符合纳入标准的神经性皮炎患者随机分为两组各30例,治疗组予丹栀逍遥散加减内服联合火针治疗,对照组予盐酸西替利嗪片口服联合地奈德软膏外用。两组均以2周为1个疗程,连续观察2个疗程后,通过患者的客观症状(皮损面积、皮损颜色、苔藓样变程度、皮损形态、肥厚程度)、主观症状(瘙痒)以及负性情绪(焦虑、抑郁)等各项指标评分进行疗效判定。结果:治疗组总有效率为83.33%,而对照组仅为36.67%(P﹤0.05)。两组患者的皮损积分在治疗第2、4周时均低于治疗前(P﹤0.05),且治疗组客观及主观症状总积分均显著低于对照组(P﹤0.05),患者的焦虑自评量(SAS)、抑郁自评量(SDS)均明显低于对照组(P﹤0.05)。结论:丹栀逍遥散加减联合火针治疗神经性皮炎临床疗效确切,其不仅能显著缓解患者的皮损状态及瘙痒程度,还能有效改善患者的负性情绪。

Objective:To observe the clinical efficacy of modified Danzhi Xiaoyao powder and fire needling on neurodermatitis with syndrome of liver depression transforming into fire and the effect on negative emotions of patients.Methods:60 cases of eligible patients with neurodermatitis were randomly divided into tow groups,30 cases in each group.The treatment group was treated with oral modified Danzhi Xiaoyao powder and fire needling,while the control group was treated with oral cetirizine hydrochloride tablets and desonide ointment for external use.For both groups,2 weeks as a course of treatment.After continuous observation for 2 courses of treatment,the clinical efficacy was evaluated by objective symptoms(such as area of skin lesion,color of skin lesion,degree of lichenoid change,shape of skin lesion,degree of hypertrophy),subjective symptoms(pruritus)and negative emotions(anxiety,depression)of patients.Results:The total effective rate of treatment group was 83.33%,while that of control group was 36.67%(P﹤0.05).At the 2nd and 4th week of treatment,in both groups,the score of skin lesion of patients was lower than that before treatment(P﹤0.05),and the total scores of objective and subjective symptoms of treatment group were significantly lower than those of control group(P﹤0.05),and the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)of patients in treatment group were significantly lower than those in control group(P﹤0.05).Conclusion:The clinical efficacy of modified Danzhi Xiaoyao powder and fire needling on neurodermatitis is definite,which can not only significantly relieve the degree of skin lesion and itching of patients,but also effectively improve the negative emotions of patients.
