肝细胞癌患者血清中G0S2表达及临床意义

摘要 目的评估血清G0/G1开关基因2(G0S2)作为肝细胞癌(HCC)潜在生物标志物的诊断价值。方法采用实时荧光定量PCR(qPCR)检测本院2016年6月至2022年6月83例HCC患者和65例健康对照的血清G0S2水平。采用受试者工作特征(ROC)曲线评价G0S2在HCC的诊断效能。分析血清G0S2水平与HCC患者临床病理特征的相关性。采用Cox风险比例回归模型分析影响HCC预后的独立危险因素。结果HCC患者的血清G0S2水平低于健康对照(0.447 vs.0.937,P<0.01)。G0S2表达水平与肿瘤分化程度(χ^(2)=5.033,P=0.025)、TNM分期(χ^(2)=7.332,P=0.007)和淋巴结转移(χ^(2)=4.878,P=0.027)有关。G0S2诊断HCC的曲线下面积为0.800(95%CI:0.728~0.871,P<0.01),最佳截断值为0.512,敏感度和特异度分别为87.69%和65.06%。G0S2高表达组HCC患者的中位总生存期为66.0个月,高于G0S2低表达组的35.0个月(χ^(2)=22.936,P<0.01)。另外,肿瘤分化程度(HR=3.045,95%CI:1.702~5.448,P<0.001)、肿瘤最大径(HR=2.047,95%CI:1.160~3.613,P=0.013)和血清G0S2表达水平(HR=0.286,95%CI:0.149~0.549,P<0.001)是影响HCC患者预后的独立危险因素。结论HCC患者血清G0S2表达水平降低,可作为肝癌诊断的潜在血清生物标志物。 Objective To evaluate the diagnostic value of G0/G1 switch gene 2(G0S2)as a potential biomarker in serum of patients with hepatocellular carcinoma(HCC).Methods Serum G0S2 level were detected in 83 HCC patients and 65 healthy controls from June 2016 to June 2022 using real-time quantitative PCR(qPCR).Receiver operating characteristic(ROC)curve analysis was applied to evaluate the value of G0S2 in the diagnosis of HCC.The correlation between serum G0S2 level and clinicopathological characteristics was analyzed.Cox proportional regression model was used to analyze the independent risk factors of prognosis in HCC.Results Serum G0S2 level in HCC patients was significantly lower than that in healthy controls(0.447 vs.0.937,P<0.01).Significant correlations were found between G0S2 expression level and some clinicopathological characteristics,including differentiation degrees(χ^(2)=5.033,P=0.025),TNM stage(χ^(2)=7.332,P=0.007)and lymph node metastasis(χ^(2)=4.878,P=0.027).The area under the curve of G0S2 in the diagnosis of HCC was 0.800(95%CI:0.728-0.871,P<0.01),and the optimal cut-off value was 0.512 with the diagnostic sensitivity of 87.69%and the specificity of 65.06%.The median overall survival(OS)of high G0S2 expression group was 66.0 months,significantly higher than 35.0 months of low G0S2 expression group(χ^(2)=22.936,P<0.01).Cox proportional hazard regression model analysis showed that differentiation degree(HR=3.045,95%CI:1.702-5.448,P<0.001),maximum diameter of the tumor(HR=2.047,95%CI=1.160-3.613,P=0.013)and serum G0S2 expression level(HR=0.286,95%CI:0.149~0.549,P=0.008)were independent risk factors for OS in HCC.Conclusion The expression level of G0S2 in serum of HCC patients is decreased,and can be used as a promising serum biomarker for the HCC diagnosis.
