摘要
目的探讨冷冻技术在治疗有生育需求的宫颈高级别瘤样病变Ⅱ级(CIN2)患者的临床应用价值。方法选择依据宫颈癌三阶梯筛查诊断为宫颈CIN2的患者120例,根据患者意愿及疾病特点行冷冻术的40例作为冷冻组,行宫颈环形电切术(LEEP)的40例作为LEEP组,行CO_(2)激光术的40例作为激光组。比较3组各项治疗结果。结果治疗后冷冻组患者、激光组患者术中出血量、宫颈挛缩率低于LEEP组(P<0.05)。LEEP组宫颈管长度为(5.45±2.03)mm,激光组为(3.31±1.82)mm,2组比较差异有统计学意义(P<0.05)。冷冻组阴道分娩率(74.07%)高于LEEP组(41.67%)、激光组(48.00%),3组比较差异有统计学意义(P<0.05)。LEEP组流产率(20.83%)高于激光组(8.00%)。结论宫颈冷冻术不影响生育功能,适用于年轻且有生育需求的CIN2患者。
Objective To investigate the application value of cryotherapy in the treatment of cervical high-grade squamous intraepithelial disease(CIN2)with reproductive need.Methods A total of 120 patients diagnosed with cervical high-grade squamous intraepithelial disease(CIN2)according to the three-step screening of cervical cancer were selected,including 40 patients undergoing cryotherapy as the freezing group,40 patients undergoing LEEP as the LEEP group and another 40 patients undergoing CO_(2)laser as the laser group.The treatment results of the three groups were compared.Results After treatment,the intraoperative blood loss in the cryotherapy group and the laser group was lower than that in the LEEP group(P<0.05).The cervical canal length of the freezing group was the same,and the cervical canal length of the LEEP group(5.45±2.03)mm was larger than that of the laser group(3.31±1.82)mm(P<0.05).The vaginal delivery rate of the cryotherapy group(74.07%)was higher than that of the LEEP group(41.67%)and the laser group(48.00%)(P<0.05).The abortion rate of 20.83%in LEEP group was higher than that in laser group(8.00%)and freezing group,and the abortion rate in freezing group was lower than that in laser group(P<0.05).Conclusion Cervical cryotherapy could not affect reproductive function and is suitable for young patients with cervical high-grade squamous epithelium.
