基于集合可视化分析系统探索针灸治疗产后压力性尿失禁的选穴规律

Law of Acupoints Selection for Treating PSUI by Acupuncture and Moxibustion Based on Set Visual Analysis System

目的:基于集合可视化分析系统与Apriori算法探索针灸治疗产后压力性尿失禁(PSUI)的选穴规律。方法:筛选符合纳入标准的针灸治疗PSUI相关文献,建立针灸处方数据库,利用R3.6.1进行集合可视化分析与Apriori算法分析针灸治疗PSUI的选穴规律。结果:本研究最终纳入文献22篇,针灸处方22个,涉及腧穴24个,腧穴出现总频次116次。其中,应用频次较高的腧穴为气海、关元、中极、足三里与三阴交;腧穴归属以任脉为主;分布以腹部、下肢及骶部为主。集合可视化分析、腧穴关联规则分析以及腧穴组合网络分析结果表明气海、关元、中极、足三里与三阴交是针灸治疗PSUI的核心腧穴。结论:针灸治疗PSUI腧穴处方以温阳化气利水为主,重点应用特定穴,选穴以病变脏腑膀胱近部(腹部及骶尾部)任脉、膀胱经腧穴为主,佐以下肢脾胃经腧穴,契合本病的病机及女子生理特点,运用集合可视化分析系统有助于总结PSUI针灸处方选穴规律。

Objective:To explore the law of acupoints selection for treating postpartum stress urinary incontinence(PSUI)by acupuncture and moxibustion based on set visual analysis system and Apriori algorithm.Methods:Relevant literature on acupuncture and moxibustion treating PSUI that met the inclusion criteria were screened to establish acupuncture prescription database.R3.6.1 set visual analysis and Apriori algorithm were used to analyze the rule of points selection in treatment of PSUI.Results:22 pieces of literature were included,involving 22 acupuncture prescriptions and 24 acupoints,and the total frequency of acupoints was 116 times.Among them,the acupoints with high frequency of application are Qihai(RN6),Guanyuan(RN4),Zhongji(RN3),Zusanli(ST36)and Sanyinjiao(SP6).Acupoints mainly belong to Ren channel and are mainly distributed in abdomen,lower limbs and sacrum.Results of visual analysis,acupoint association rule analysis and acupoint combination network analysis showed that RN6,RN4,RN3,ST36 and SP6 were the core acupoints for treating PSUI.Conclusion:The prescriptions of acupuncture and moxibustion treatment for PSUI are mainly to warm Yang,promote the flow of Qi and promote diuresis,with specific acupoints.The selected acupoints around abdomen and sacrococcygealare are mainly from Ren channel and Bladder meridian of Foot-Taiyang,and the acupoints on lower limbs from Spleen meridian and Stomach meridian are used as auxiliary points,which are consistent with the pathogenesis of this disease and the physiological characteristics of women.The application of set visual analysis system is helpful to summarize the law of acupoints selection in treatment of PSUI.
