摘要
目的:探讨基于Cox健康行为互动模式(interaction model of client health behavior,IMCHB)的康复护理对冠心病慢性心力衰竭(chronic heart failure,CHF)患者希望水平、自我管理行为和康复效果的影响。方法:冠心病CHF患者102例随机分为对照组(n=51)与观察组(n=51)。对照组予以常规护理,观察组在常规护理基础上予以基于Cox-IMCHB理论的康复护理。干预6个月后进行效果评价,比较2组希望水平、健康素养评分、自我管理行为评分、心功能[左室射血分数(left ventricular ejection fraction,LVEF)、6 min步行距离(6-minute walking distance,6MWT)]和生活质量[明尼苏达心力衰竭生活质量(Minnesota heart failure quality of life,MLHFQ)]评分的变化。结果:干预后,观察组希望水平评分、健康素养评分和自我管理行为评分均显著高于对照组(均P<0.05)。干预后,观察组LVEF、6MWT均高于对照组(均P<0.05)。干预后,观察组MLHFQ评分低于对照组(P<0.05)。结论:基于Cox-IMCHB理论的康复护理能够提高冠心病CHF患者希望水平,增强其健康素养和自我管理,改善心功能和生活质量。
Objective:To explore the effect of rehabilitation nursing based on Cox interaction model of client health behavior(IMCHB)on hope level,self-management behavior,and rehabilitation effect in patients with chronic heart failure(CHF).Methods:A total of 102 CHF patients were randomly divided into a control group(n=51)and an observation group(n=51).The control group was given routine nursing,and the observation group was given rehabilitation nursing based on Cox-IMCHB theory on the basis of routine nursing.The effect was evaluated after 6 months of intervention.The changes of hope level,health literacy score,self-management behavior score,cardiac function[left ventricular ejection fraction(LVEF),6-minute walking distance(6MWT)]and quality of life[Minnesota heart failure quality of life(MLHFQ)]scores were compared between the 2 groups.Results:After intervention,the scores of hope level,health literacy and self-management behavior in the observation group were significantly higher than those in the control group(all P<0.05).After the intervention,LVEF and 6MWT in the observation group were higher than those in the control group(both P<0.05).After intervention,the MLHFQ score in the observation group was lower than that in the control group(P<0.05).Conclusion:Rehabilitation nursing based on Cox-IMCHB theory can improve the hope level in patients with coronary heart disease and CHF,enhance their health literacy and selfmanagement,and improve cardiac function and quality of life.
