暴发性1型糖尿病合并Tako-Tsubo综合征1例病例报道及文献复习

Fulminant type 1 diabetes complicated with Tako-Tsubo syndrome:A case report and literature review

回顾性分析1例暴发性1型糖尿病(fulminant type 1 diabetes mellitus,FT1DM)合并Tako-Tsubo综合征(Tako-Tsubo syndrome,TTS)患者的临床特点并结合相关文献总结分析该病诊治过程。本例患者以胃肠道症状起病,快速进展为酮症酸中毒,入院后经实验室检查明确为FT1DM,其中根据心电图、心肌酶、肌钙蛋白的改变,结合患者在院期间病情变化及随访结果最终诊断为FT1DM合并TTS。FT1DM可引起多脏器损伤,其中心肌损伤尤为重要,如果不能及时明确病因,容易误导临床诊治,导致患者不良结局的发生。

To retrospectively analyze the clinical characteristics of a patient with fulminant type 1 diabetes mellitus(FT1DM)complicated with Tako-Tsubo syndrome(TTS),the diagnosis and treatment process of this disease were summarized combined with related literature.This patient started with gastrointestinal symptoms and rapidly progressed to ketoacidosis.After admission,laboratory tests confirmed FT1DM.The changes in electrocardiogram,myocardial enzymes and troponin,combined with the changes of the patient’s condition during hospital stay and the follow-up results,the patient was finally diagnosed as FT1DM combined with TTS.FT1DM can cause multiple organ injury,especially myocardial injury.If the etiology is not identified in time,it is easy to mislead clinical diagnosis and treatment,leading to the occurrence of adverse outcomes.
