Study on the influence of different injection velocities on the image quality of coronary CTA examination

摘要 目的探究不同注射流速对冠脉CT血管造影(CTA)检查图像质量的影响。方法选择2022年3月-2023年3月于南昌大学第二附属医院行冠脉CTA检查的88例患者作为研究对象,采用随机数字表法分为两组,每组各44例。所有患者检查前均使用20G套管针对患者右侧肘静脉进行穿刺,研究组采用3.0-4.5ml/s的流速注射对比剂,对照组采用4.5-5.5ml/s的流速注射对比剂。对比两组主动脉脉根CT值、图像质量评分、伪影分级、室间隔显示评分及外渗发生率。结果研究组主动脉脉根CT值低于对照组,图像质量评分及室间隔显示评分均高于对照组,有统计学差异(P<0.05)。研究组伪影分级优于对照组,有统计学差异(P<0.05)。研究组外渗发生率为0,对照组外渗发生率为6.82%,但两组对比无统计学差异(P>0.05)。结论选择3.0-4.5ml/s的注射流速在冠脉CTA检查中可获得较高的图像质量,同时有效减少对比剂外渗发生率,大大降低血管壁损伤发生风险,为临床诊疗提供可靠的影像学参考,值得推广应用。 Objective To investigate the effect of different injection velocities on the image quality of coronary CT angiography(CTA).Methods 88 patients who underwent coronary CTA examination in our hospital from March 2022 to March 2023 were selected as the study subjects.They were divided into two groups using a random number table method,with 44 patients in each group.Before examination,all patients were punctured with a 20G cannula through the right elbow vein of the patient.The study group was injected with contrast agent at a flow rate of 3.0-4.5ml/s,while the control group was injected with contrast agent at a flow rate of 4.5-5.5ml/s.Compare the CT value,image quality score,artifact grade,ventricular septal display score,and the incidence of extravasation between the two groups.Results The CT value of aortic root in the study group was lower than that in the control group,and the image quality score and ventricular septal display score were higher than those in the control group,with a statistically significant difference(P<0.05).The artifact grading in the study group was better than that in the control group,with a statistically significant difference(P<0.05).The incidence of extravasation was 0 in the study group and 6.82%in the control group,but there was no significant difference between the two groups(P>0.05).Conclusion Choosing an injection flow rate of 3.0-4.5ml/s in coronary CTA can obtain high image quality,effectively reduce the incidence of contrast agent extravasation,greatly reduce the risk of vascular wall injury,and provide reliable imaging reference for clinical diagnosis and treatment,which is worth promoting and applying.
