摘要
目的探讨单采血小板捐献次数对献血者铁代谢及甲状旁腺功能的影响。方法选取嘉峪关市中心血站2022年1—12月91例单采血小板无偿献血者,根据不同的血小板捐献次数分为四组,初次献血者27例,记为A组;实行《献血者健康检查要求GB18467-2011》以来,至今累积献血2~30次献血者25例,记为B组;31~60次献血者20例,记为C组;超过60次献血者19例,记为D组。采用全自动血细胞分析仪检测不同组别单采献血者血浆中的血红蛋白(hemoglobin,Hb)、铁蛋白(serum ferritin,SF)、转铁蛋白(transferrin,TRF)、可溶性转铁蛋白受体(soluble transferrin receptor,sTfR)水平,全自动酶联免疫分析仪Microlab FAME检测四组的甲状旁腺激素(parathyroid hormone,PTH)水平。结果四组的Hb水平比较,差异无统计学意义(P>0.05);B、C、D组的SF分别为(21.58±8.11)μg/L、(15.58±7.68)μg/L、(11.52±9.64)μg/L、均低于A组的(26.88±7.60)μg/L,差异有统计学意义(P<0.05);B、C、D组的TRF分别为(2.44±0.26)mg/L、(3.24±0.51)mg/L、(3.64±0.66)mg/L、均高于A组的(2.15±0.34)mg/L,差异有统计学意义(P<0.05);B、C、D组的sTfR分别为(2.97±0.42)mg/L、(3.21±0.78)mg/L、(3.72±0.73)mg/L、均高于A组的(2.44±0.52)m g/L,差异有统计学意义(P<0.05);B、C、D组的PTH分别为(79.28±11.55)pg/L、(91.68±12.64)pg/L、(135.55±20.66)pg/L、均高于A组的(56.77±10.64)pg/L,差异有统计学意义(P<0.05)。结论血小板捐献虽然是一项重要的献血方式,但也会对献血者的铁代谢和甲状旁腺功能造成影响。因此,在进行血小板捐献时,需要关注这些问题,以保障献血者的健康和安全。
Objective To investigate the effect of platelet donation frequency on iron metabolism and parathyroid function in blood donors.Methods A total of 91 unpaid platelet donors who were admitted to Jiayuguan Central Blood Station from January to December 2022 were selected and divided into 4 groups based on different platelet donation times.27 first-time donors were recorded as Group A.Since the implementation of the"Health Examination Requirements for Blood Donors GB18467-2011",there have been a total of 25 blood donors who have donated 2-30 times,recorded as Group B,20 cases of 31-60 blood donors were recorded as Group C,19 cases with over 60 blood donors were recorded as Group D.The levels of hemoglobin(Hb),serum ferritin(SF),transferrin(TRF),and soluble transferrin receptor(sTfR)in the plasma of different groups of single blood donors were detected using a fully automated hematology analyzer(Sysmex XN-9000).The levels of parathyroid hormone(PTH)in the four groups were measured using a fully automated enzyme-linked immunosorbent assay(Microlab FAME).Results There was no statistically significant difference in Hb levels among four groups(P>0.05).The SF values of Group B,Group C,and Group D were(21.58±8.11)μg/L,(15.58±7.68)μg/L,(11.52±9.64)μg/L,all lower than group A(26.88±7.60)μg/L,there was a statistically significant difference between groups(P<0.05).The TRF of Group B,Gr o u p C,a n d G r o u p D w e r e(2.44±0.26)mg/L,(3.24±0.51)mg/L,and(3.64±0.66)mg/L,respectively,which were higher than those of Group A(2.15±0.34)mg/L,with statistically significant differences between the groups(P<0.05).The sTfR of Group B,Group C,and Group D were(2.97±0.42)mg/L,(3.21±0.78)mg/L,and(3.72±0.73)mg/L,respectively,higher than that of Group A(2.44±0.52)mg/L,with statistically significant differences between the groups(P<0.05).The PTH levels of Group B,Group C,and Group D were(79.28±11.55)pg/L,(91.68±12.64)pg/L,and(135.55±20.66)pg/L,respectively,which were higher than those of Group A(56.77±10.64)pg/L.The difference between the groups was statistically significant(P<0.05).Conclusion Although platelet donation is an important blood donation method,it can also affect the iron metabolism and parathyroid function of blood donors.Therefore,when conducting platelet donation,it is necessary to pay attention to these issues to ensure the health and safety of blood donors.
