基于动态评估的语言康复训练在脑卒中后失语症患者中的应用

Application of Language Rehabilitation Training Based on Dynamic Assessment in Patients With Aphasia After Cerebral Stroke

目的探讨基于动态评估的语言康复训练对脑卒中后失语症患者的疗效与生活质量的影响。方法回顾性分析厦门大学附属第一医院2020年1月—2022年8月的106例脑卒中后失语症患者,其中53例为对照组,施行常规康复护理与功能训练;另53例为试验组,在前组基础上施行基于动态评估的语言康复训练。统一由1名护理人员于训练前后采用汉语失语症检查法、日常生活交流能力评定法(communication abilities of daily living,CADL)、汉化脑卒中失语症生活质量量表(stroke aphasia quality of life scale,SAQOL-39g)来评价两组患者的语言能力、日常生活交流能力以及生活质量,比较组间差异。结果两组训练前的语言能力、CADL评分及生活质量评分比较,差异无统计学意义(P>0.05),训练后各评分均呈上升趋势,其中以试验组的升高幅度更大,与对照组比较,差异有统计学意义(P<0.05)。结论采用基于动态评估的语言康复训练可显著提高脑卒中后失语症患者的口语输出能力和语言交流能力,改善整体生活质量,安全性高,应用效果肯定。

Objective To discuss the influence of dynamic evaluation-based language rehabilitation training on the curative effects and life quality of cerebral stroke patients with aphasia.Methods A retrospective analysis was performed on 106 patients with post-stroke aphasia from January 2020 to August 2022 in the First Affiliated Hospital of Xiamen University.53 cases receiving the routine rehabilitation nursing and function training were included in the control group,53 cases receiving the dynamic evaluation-based language rehabilitation training were included in the trial group.Before and after care,a designated nursing staff would evaluate the patient’s language competence,daily living communication ability and life quality through China rehabilitation research center aphasia examination,communication abilities of daily living(CADL)and stroke aphasia quality of life scale(SAQOL-39g).Results Before training,there were no significant between-group differences about the patient’s language competence,CADL and life quality scores(P>0.05).After training,the patient’s language competence,CADL and life quality scores in the trial group were higher than control group,with statistically significant difference(P<0.05).Conclusion The use of dynamic assessment based speech rehabilitation training can significantly improve the oral output ability and language communication ability of patients with aphasia after stroke,improve the overall quality of life,high safety,and positive application effect.
