哮喘患儿血清CircZNF652的表达水平及其临床意义

摘要 目的探讨哮喘患儿血清环状RNA锌指蛋白652(CircZNF652)的表达水平及其临床意义。方法选取105例哮喘患儿作为观察组,根据临床表现将其分为急性发作期组(57例)和慢性缓解期组(48例),另选取80例健康儿童作为对照组。比较观察组与对照组之间、急性发作期组和慢性缓解期组之间的血清CircZNF652、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)的表达水平,以及肺功能指标[1秒用力呼气容积(FEV_(1))、呼气峰值流速(PEF)、FEV_(1)/用力肺活量(FVC)值]。采用Pearson检验分析观察组血清CircZNF652表达水平与肺功能指标及血清TNF-α、IL-6表达水平的相关性。绘制受试者工作特征曲线分析血清CircZNF652表达水平对儿童哮喘的诊断价值。采用多因素Logistic回归模型分析儿童哮喘急性发作的影响因素。结果观察组FEV_(1)、PEF、FEV_(1)/FVC值均低于对照组,而血清CircZNF652、TNF-α、IL-6表达水平均高于对照组(均P<0.05);急性发作期组FEV_(1)、PEF、FEV_(1)/FVC值均低于慢性缓解期组,而血清CircZNF652、TNF-α、IL-6表达水平均高于慢性缓解期组(均P<0.05)。观察组血清CircZNF652表达水平与FEV_(1)、PEF、FEV_(1)/FVC值均呈负相关,与血清TNF-α、IL-6表达水平均呈正相关(均P<0.05)。血清CircZNF652表达水平诊断儿童哮喘的曲线下面积为0.741,灵敏度和特异度分别为75.24%、67.50%,最优截断值为1.12。FEV_(1)、PEF、FEV_(1)/FVC值,以及血清TNF-α、IL-6、CircZNF652表达水平均是哮喘患儿急性发作的影响因素(均P<0.05)。结论哮喘患儿血清CircZNF652表达水平上调,并与哮喘急性发作有关,且与肺功能指标呈负相关,与血清TNF-α及IL-6表达水平呈正相关。检测血清CircZNF652表达水平有助于儿童哮喘的病情判定。 Objective To investigate the expression of serum circular RNA zinc finger protein 652(CircZNF652)in children with asthma and its clinical significance.Methods A total of 105 children with asthma were selected as observation group,and they were assigned to acute exacerbation group(57 cases)or chronic remission group(48 cases)according to clinical manifestations,another 80 healthy children were selected as control group.The expressions of serum CircZNF652,tumor necrosis factorα(TNF-α),and interleukin 6(IL-6),as well as pulmonary function indices with respect to forced expiratory volume in one second(FEV_(1)),peak expiratory flow(PEF),and FEV_(1)/forced vital capacity(FVC)value were compared between the observation group and the control group,and between the acute exacerbation group and the chronic remission group.The correlation of serum CircZNF652 expression with pulmonary indices and serum TNF-αand IL-6 expressions in the observation group was analyzed by using the Pearson test.The diagnostic value of serum CircZNF652 expression on children with asthma was analyzed by drawing receiver operating characteristic curve.The multivariate Logistic regression model was employed to analyze the influencing factors for acute exacerbation of children with asthma.Results The observation group exhibited lower FEV_(1),PEF,and FEV_(1)/FVC value,whereas higher expressions of serum CircZNF652,TNF-α,and IL-6 as compared with the control group(all P<0.05).The acute exacerbation group yielded lower FEV_(1),PEF,and FEV_(1)/FVC value,whereas higher expressions of serum CircZNF652,TNF-α,and IL-6 as compared with the chronic remission group(all P<0.05).Serum CircZNF652 expression negatively correlated with FEV_(1),PEF,and FEV_(1)/FVC value in the observation group,and positively correlated with serum TNF-αand IL-6 expressions in the observation group(all P<0.05).The area under the curve of serum CircZNF652 expression for diagnosing children with asthma was 0.741,the sensitivity and specificity were 75.24%and 67.50%,respectively;in addition,the optimal cut-off value was 1.12.FEV_(1),PEF,FEV_(1)/FVC value,and expressions of serum TNF-α,IL-6,and CircZNF652 were the influencing factors for acute exacerbation in children with asthma(all P<0.05).Conclusion Serum CircZNF652 expression in children with asthma is up-regulated,and related to acute exacerbation of asthma;furthermore,it negatively correlates with pulmonary function indices,and positively correlated with expressions of serum TNF-αand IL-6.Therefore,the detection of serum CircZNF652 expression is helpful to determine the disease condition of children with asthma.
