认知行为干预在绝经激素治疗围绝经期综合征中的应用效果及对患者负性情绪的影响

Application effect of cognitive behavioral therapy in menopausal hormone treatment perimenopausal period syndrome and its influence on patients' negative emotions

目的研究认知行为干预(CBT)在绝经激素治疗(MHT)围绝经期综合征(PPS)中的应用效果。方法将宝鸡市妇幼保健院收治的186例PPS患者随机分为常规组和CBT组,各93例。两组均行MHT,常规组实施常规护理,CBT组在常规组基础上实施CBT。比较两组的干预效果。结果CBT组的治疗总有效率高于常规组,差异具有统计学意义(P<0.05)。干预后,CBT组的黄体生成素(LH)、促卵泡激素(FSH)水平低于常规组,雌二醇(E2)水平高于常规组(P<0.05)。干预后,CBT组的血管舒缩症状、生理、性生活和心理评分低于常规组(P<0.05)。干预后,CBT组的焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于常规组,PPS知识掌握程度评分高于常规组(P<0.05)。结论CBT可提高MHT对PPS患者的治疗效果,改善性激素水平和生活质量,减轻负性情绪,值得推广。

Objective To study the application effect of cognitive behavioral therapy(CBT)in menopausal hormone treatment(MHT)perimenopausal period syndrome(PPS).Methods A total of 186 PPS patients admitted in Baoji Maternal and Child Health Care Hospital were randomly divided into routine group and CBT group,with 93 cases in each group.Both groups implemented MHT,the routine group used routine nursing,and the CBT group used CBT on the basis of the routine group.The imtervention effects of the two groups were compared.Results The total effective rate of treatment in the CBT group was higher than that in the routine group,and the difference was statistically significant(P<0.05).After intervention,the levels of luteinizing hormone(LH)and follicle stimulating hormone(FSH)in the CBT group were lower than those in the routine group,and the level of estradiol(E2)was higher than that in the routine group(P<0.05).After intervention,the scores of vasomotor symptoms,physiology,sexual life and psychology in the CBT group were lower than those in the routine group(P<0.05).After intervention,the scores of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)in the CBT group were lower than those in the routine group,and the score of PPS knowledge mastery was higher than that in the routine group(P<0.05).Conclusion CBT can improve the therapeutic effect of MHT on PPS patients,improve sex hormone levels and quality of life,reduce negative emotions,which is worthy of promotion.
