门诊化疗一体化管理模式的建立

Integrated Management Model of Outpatient Chemotherapy Establishment

目的门诊化疗节省患者等待时间,减少医疗费用,提高医疗资源利用效率,是一种更有优势的治疗模式,但这种新型模式没有经验可循,因此探索和分享新型、规范、便捷、优化的门诊治疗模式尤为重要。方法本文从哈尔滨医科大学附属肿瘤医院门诊化疗治疗模式分享、科室运行现状以及如何优化科室建设、节省医保开支、改变患者观念的角度介绍了门诊化疗管理模式。结果哈尔滨医科大学附属肿瘤医院门诊化疗共配备120张沙发椅,有专职医护团队,主管医生评估患者符合门诊治疗标准后下达医嘱,门诊化疗科室配合执行并由专职医护人员负责治疗安全。黑龙江省患者门诊治疗均可经医保报销,同时具备标准的门诊病历书写模式,以及统一的质控规范。2020年4月—2022年6月,门诊化疗科共收治静脉点滴患者65615人次,皮下注射32483人次,处理不良反应平均24次/月。结论哈尔滨医科大学附属肿瘤医院门诊化疗成熟、标准,符合当前中国医院实际情况模式。

Objective Outpatient chemotherapy saves patients'waiting time,reduces medical costs,and improves the efficiency of medical resources utilization.It is a more advantageous treatment mode,but has no experience to follow.The purpose is to explore and share new,standardized,convenient,and optimized outpatient treatment modes.Methods This paper shares the outpatient chemotherapy treatment model in Harbin Medical University cancer hospital,including the current status,the way to optimize department construction,saving health insurance expenditure,and change patients'opinion.Results Outpatient chemotherapy department of Harbin Medical University cancer hospital is equipped with 120 couches,and a full-time medical team.The doctor in charge assesses the patients to meet the criteria for outpatient treatment and gives medical advice,then the outpatient chemotherapy department cooperates with the implementation and responsible for the safety of treatment by the full-time medical and nursing staff.The outpatient treatment of patients in Heilongjiang province can be reimbursed by medical insurance,and there is a standard outpatient medical record writing mode,as well as a unified quality control norms.From April 2020 to June 2022,t h e o utpa t i ent chemotherapy department admitted a total of 65615 patients with intravenous drips,and 32483 subcutaneous injections.The department dealt with adverse reactions on average of 24 times per month.Conclusion Outpatient chemotherapy in Harbin Medical University cancer hospital is mature,standardized,and in line with the current model of the actual situation in Chinese hospitals.
