巨大复杂性宫颈黏膜下肌瘤1例

A case of giant and complex cervical submucous myoma

宫颈肌瘤为特殊部位的子宫肌瘤,巨大的宫颈肌瘤罕见,临床处理难度较大,目前尚无统一诊疗规范。对于青年患者,需考虑保护生育功能,完全切除肌瘤的同时还要着眼于子宫形态及功能的恢复,完成宫腔、宫颈管及子宫成形术。本文报道1例34岁未孕女性因巨大宫颈黏膜下肌瘤(30 cm×20 cm×20 cm)行经腹子宫肌瘤切除术,通过阐述其诊治过程,探讨对于巨大宫颈肌瘤术前预处理的方式、保护子宫形态和功能手术预案的选择,提供对于青年女性复杂肌瘤的诊治思路。

Cervical myoma is a special part of uterine myoma.Giant cervical myoma is rare and difficult to treat clinically.At present,there is no unified diagnosis and treatment standard.For young patients,it is necessary to consider the protection of reproductive function,which focuses on the recovery of uterine morphology and function while completely removing myoma,and completes the uterine cavity,cervical canal and hysteroplasty.This paper reports a case of 34-year-old non-pregnant woman who underwent transabdominal myomectomy for giant cervical submucous myoma(30 cm×20 cm×20 cm).By elaborating the process of diagnosis and treatment,this paper discusses the way of preoperative pretreatment for giant cervical myoma,the choice of surgical plan to protect shape and function of the uterus,and provides the ideas for the diagnosis and treatment of young women with complex myoma.
