摘要
目的探讨儿科病例教学在八年制临床医学生见习临床思维培训中的应用。方法2022年1月3日-3月31日在医院儿科见习的2018级八年制临床医学生55名为研究对象。随机分为典型病例教学组28名,拓展病例教学组27名。自病例库选取病例,典型病例组给与典型病例教学,拓展病例组给与典型病例教学加拓展病例教学。采用符号秩和检验分析病例分析成绩,学生问卷调查方法分析病例教学认可度。结果两组病例分析成绩均较理想[90分&85分]。拓展病例组中病例特点及鉴别诊断成绩明显高于典型病例组[25(25.25)分&25(20.25)分],[15(10,15)分&10(5,15)分],差异有显著性(P<0.05)。96%的学生认为病例教学可有效巩固理论知识,100%认为能提高临床思维能力,98%认为能激发对新进展探索的兴趣。结论应用典型病例教学可有效培养八年制医学生见习临床思维能力,增加拓展病例教学,可进一步增加教学效果。病例教学方式获得学生的广泛认可。
Objectives To explore the effectiveness of pediatric case teaching in clinical thinking training in trainee teaching of eight-year internship students.Methods 55 eight-year clinical students who were interns in the pediatric department from 3rd January,2022 to 31st March,2022 were enrolled as the research group.These students were randomly divided into two groups:classical cases training group and advanced cases training group.The classical cases and advanced cases were selected in pediatric case database.28 students in the classical cases group were taught with classical cases learning,and 27 students in the advanced cases group were additionally taught with more advanced cases.The Wilcoxon test and the questionnaires are respectively used to analyze the students’case analysis scores and their satisfaction with the teaching.Results The scores of case analysis in both groups were satisfactory[90&85].The scores of case characteristics and differential diagnosis in the advanced cases group were significantly higher than those in the classical cases group[25(25.25)&25(20.25)],[15(10,15)&10(5,15)].The difference was significant(P<0.05).96%of students thought that case-base teaching was effective in consolidating theoretical knowledge,100%thought it improved clinical thinking skills and 98%thought it stimulated an interest in exploring new advances.Conclusions The application of classical case teaching could effectively develop the clinical thinking skills of eight-year medical students in their internship,and the addition of advanced case teaching could further increase the teaching effect.The case teaching method had been widely recognized by students.
