摘要
数十年来,糖化血红蛋白(HbA1c)一直是评价血糖控制的金标准,但是其存在局限性,特别是缺乏血糖变异性或低血糖风险的信息。葡萄糖在目标范围内时间(TIR)被定义为个体血糖在目标范围内时间的百分比,是一种来源于持续血糖监测(CGM)的简单直观的指标,可反映血糖控制整体情况。不断有研究证实TIR与HbA1c密切相关,并与糖尿病并发症的风险相关,在糖尿病及其慢性并发症的评估防治中发挥越来越重要的作用。文章综述了TIR与糖尿病慢性并发症之间的关系,即多数研究提示TIR的降低与糖尿病慢性并发症风险增高相关,进一步探讨了TIR的临床应用价值和广泛应用前景。
Glycosylated hemoglobin(HbA1c)has been the gold standard for evaluating glucose control for decades,but during its use limitations have been found,particularly in the lack of information on glucose variability or risk of hypoglycemia.Time in range(TIR)of glucose,defined as the percentage of time that an individual spends within the target glucose range,is a simple and intuitive measure derived from continuous glucose monitoring(CGM)which reflects overall glucose control.In recent years,it has been proved that TIR is closely related to HbA1c and is associated with the risk of diabetic complications,and plays an increasingly important role in the evaluation and prevention of diabetes and its chronic complications.The paper reviewed the association between TIR and chronic complications of diabetes,that is,most studies suggested that a reduction in TIR is associated with an increased risk of chronic complications of diabetes,and further discussed the clinical value and extensive application prospect of TIR.
